    Detection of serum ischemia-modified albumin in patients with early traumatic craniocerebral injury

    Objective To research the dynamic changes and clinical significance of serum ischemia-modified albumin(IMA) in patients with early traumatic craniocerebral injury.   Methods A total of 90 patients with traumatic craniocerebral injury were classified into 3 groups according to the Glasgow Coma scale(GCS) scores [score 13-15:mild injury group(29 cases),score 9-12:medium injury group(29 cases) and score 3-8:severe injury group(32 cases)],and 36 healthy subjects were enrolled as healthy controls. The levels of serum IMA,C reactive protein(CRP) and myeloperoxidase(MPO) were determined in patient group at 3,6,12,24,48 and 72 h after injury and in healthy control group. Results The levels of serum IMA at 3-72 h after injury in patients with traumatic craniocerebral injury were significantly higher than those in healthy control group(P<0.01) and increased with the degree of injury,and the levels of serum IMA in mild injury group were lower than those in medium and severe injury groups(P<0.01). The levels of serum IMA in early injury had no change with the development of the disease(P>0.10),and there was a positive correlation with CRP and MPO. Conclusions The levels of serum IMA are significantly increasing continuously because of oxygen lack and inflammation reaction in patients with early traumatic craniocerebral injury,and are related positively with the severity of disease. IMA has the significance for the severity evaluation,secondary craniocerebral injury and therapy in patients with early traumatic craniocerebral injury.
    Establishment and evaluation of detection limits and function sensitivity of high-sensitivity assays for cardiac troponin I

    Objective To establish and evaluate the limit of blank(LoB),the limit of detection(LoD) and the limit of quantitation(LoQ) of high-sensitivity assays for cardiac troponin I(cTnI) on chemiluminescence immune analyzer.   Methods According to the document EP17-A published by Clinical and Laboratory Standards Institute(CLSI),cTnI blank samples and serial low concentration samples were detected by BECKMAN-COULTER DXI800 chemiluminescence immune analyzer(DXI800). Based on the data distribution,by relevant statistical methods,the LoB,LoD and LoQ of cTnI were calculated. Simultaneously,by the traditional methods,the low limit of detection(LLD),biological limit of detection(BLD) and function sensitivity(FS) were established. Results According to CLSI EP17-A document,the LoB was 3.5ng/L,the LoD was 11.5ng/L,and the LoQ was 30 ng/L. The LLD was 5.4 ng/L,the BLD was 10-20 ng/L,and the FS was 30 ng/L by the traditional methods. Conclusions The LoB and LLD are both lower than the sensitivity performance of the manufacturer declaration. The analysis sensitivity of cTnI by DXI800 is validated. The established LoB,LoD,LoQ and FS of cTnI in the laboratory provide more accurate and reliable results.
    Analysis on the correlation of homocysteine levels and the traditional risk factors in patients with cerebral infarction

    Objective To study the changes of homocysteine(Hcy) levels in patients with cerebral infarction in order to investigate whether the increase of Hcy is an independent risk factor for cerebrovascular disease,and to observe Hcy and other traditional risk factors for guiding clinical treatment and prevention.   Methods Hcy was determined by cyclic enzymatic method. A total of 292 patients with cerebral infarction were enrolled,and 80 healthy subjects were as control group. The blood pressure,fasting glucose(Glu),triglyceride(TG),total cholesterol(TC),lipoprotein(a) [Lp(a)] and uric acid(UA) were determined. The correlation of traditional indicators and Hcy was analyzed by correlation coefficient(r) with regression analysis. The results of patients with cerebral infarction and control group were analyzed. Results Hcy level was(18.19±7.26)μmol / L in patients with cerebral infarction. The Hcy level in control group was(11.23±5.46) μmol / L(P<0.05). If the Hcy level > 20 μmol/L was settled as high Hcy hematic disease diagnosis standard,the incidence rate in patients with cerebral infarction was 20.5%,and the incidence rate in control group was only 1.2% with statistical difference(P<0.05). In cerebral infarction patients,the Hcy level of males was(19.84±7.87) μmol / L ,which was significantly higher than that of females [(12.38±7.42)μmol / L](P<0.05). Age,smoking history,fasting Glu,TG and TC had no statistical significance(P>0.05). In a variety of traditional risk factors for cerebral infarction,the correlation of UA and Lp(a) with Hcy was positive,and r were 0.040 and 0.044(P<0.05). Age,diastolic blood pressure,Glu,TG and TC had no correlation with Hcy levels(P>0.05). Conclusions High Hcy hyperlipidemia is an independent risk factor for cerebral infarction,UA,Lp(a) and Hcy are closely related.

    Objective To investigate the application significance of reticulocyte hemoglobin concentration(CHr) and hypochromic red blood cell percentage(Hypo%) for the screening of iron deficiency anemia(IDA)during pregnancy.   Methods The levels of red blood cell(RBC),hemoglobin(Hb),mean corpuscular volume(MCV),mean corpuscular hemoglobin(MCH),mean corpuscular hemoglobin concentration(MCHC),Hypo%,red cell volume distribution width(RDW),CHr and the percentage of reticulocyte(Ret%) in peripheral blood of pregnant women were determined by ADVIA2120 hematology analyzer. The levels of serum iron(SI) were measured simultaneously. Results The levels of CHr,RBC,Hb,MCV,MCH,MCHC,Hypo%,RDW and SI in IDA group had statistical significance compared with those in non-IDA group and normal group(P<0.05). Ret% had no statistical significance(P>0.05). The levels of CHr and Hypo% in IDA group were(26.8±3.4)pg and(7.5±3.7)%,which had statistical significance with those of non-IDA group [CHr(31.9±2.4)pg and Hypo%(1.6±1.6)%] and normal group [CHr(33.9±2.2) pg and Hypo%(0.7±0.8) %,P<0.01]. When the cut-off value was 29.0 pg,the sensitivity of CHr was 81.0%,the specificity was 93.0%,and the area under receiver operating characteristic(ROC) curve was 0.929. When the cut-off value was 6%,the sensitivity of Hypo% was 78.0%,the specificity was 95.0%,and the area under ROC curve was 0.933. Conclusions The determination of CHr and Hypo% can be used as sensitive and early indices in the diagnosis of IDA in pregnant women. The simultaneous detection of CHr and Hypo% can provide scientific reference for nutrition intaking in pregnant women.

    Objective To investigate the clinical significance of bone metabolism markers in patients with osteoporosis.   Methods Bone mineral densities(BMD) of vertebral column and femur were measured by double functional X-ray digital bone density meter. A total of 74 patients were classified into 3 groups according to BMD:normal control(19 cases),bone-mass-reduction(25 cases) and osteoporosis(30 cases). Serum parathyroid hormone(PTH),beta-Crosslaps(β-CTX),osteocalcin(N-MID) and 25-OH Vitamin D3 levels were detected by electrochemiluminescence immunoassay. Results β-CTX and N-MID levels of osteoporosis group were significantly higher than those of normal control group and bone-mass-reduction group,and the difference had statistical significance(P<0.05). PTH and 25-OH Vitamin D3 levels had no statistical significance among the 3 groups(P>0.05). Conclusions There is a close relationship between bone metabolism markers and osteoporosis. Serum bone turnover markers of β-CTX and N-MID are good indicators in the diagnosis of osteoporosis and fracture risk prediction.
    Establishment of the reference range of serum superoxide dismutase measured by pyrogallol autoxidation method in healthy adults

    Abstract:Objective To establish the reference value of superoxide dismutase(SOD) measured by pyrogallol autoxidation method in healthy adults. Methods 1640 clinical serum samples for SOD were detected by pyrogallol autoxidation method in healthy adults,and the results were statisticed according to sex and age. Results The distribution of serum SOD was normal in health adults,and the levels were negatively correlated with ages(r=-0.541,P<0.01).There was a significant difference in the levels of SOD between males and females(P<0.01).The reference range in males: 147.6~218.4 U/mL in ≤40-year-old group,131.4~207.8 U/mL in 40 to 60-year-old group, 98.1~190.9 U/mL in >60-year-old group.And in females: 142.0~205.2 U/mL in ≤40-year-old group,126.4~194.4U/mL in 40 to 60-year-old group, 95.5~187.5 U/mL in >60-year-old group. Conclusions The reference range of serum SOD are inconsistent in different healthy people,it is essential to establish the rational reference value by respective detection system and detection method in different clinical laboratory.

    Objective To study the abnormality and influence factors of alanine aminotransferase(ALT) in the fresh students from some universities of Gansu province 2008-2010 year,and to investigate the prevention from hepatitis in universities.   Methods A total of 6 852 fresh students were enrolled for physical examination in some universities of Gansu province 2008-2010 year. The serum ALT levels and influence factors were analyzed.  Results The positive rate of ALT in 6 852 fresh students was 2.30%,and there was no statistical significance between male and female students(P>0.05). The positive rate of hepatitis surface antigen(HBsAg) was 3.40%,and there were statistical significances between male and female students(P<0.05). Among the influence factors for ALT abnormality,the positivity of HBsAg was the highest,accounted for 19.74%,and fatty liver(18.47%) and tiredness(17.83%). The number of males with fatty liver was more than that of females slightly with no statistical significance(P>0.05),however,tiredness in females accounted markedly higher than that in males(P<0.05). The results of multivariate unconditioned Logistic regression analysis showed that the influence factors for ALT abnormality included virus infection,fatty liver,tiredness,family genetic history,oral nutriments and taking other medicines.  Conclusions The positive rate of HBsAg in fresh students from some universities of Gansu province is lower than that of other same aged population through whole country. The students with ALT abnormality could be cured by changing living habits combined with medicine treatment.

    Objective To investigate the clinical significance of anticytoplasmic autoantibodies. Methods A total of 164 patients with anticytoplasmic autoantibody positive were retrospected.   Results In the 164 cases with anticytoplasmic autoantibody positive,14 cases(9%)were fibrillar pattern,and 150 cases(91%)were speckled pattern. Anticytoplasmic autoantibodies were singly positive in 79 cases(48%),and the specific antibodies were detected in 9 cases. Anticytoplasmic and antinuclear autoantibodies were both positive in 85 cases(52%),and the specific antibodies were detected in 53 cases. Statistical significance existed between the detection rates of the specific antibodies in the 2 groups(P<0.01). A total of 99 cases(60%) were diagnosed as autoimmune diseases,in which 68 cases were positive with both anticytoplasmic and antinuclear autoantibodies. A total of 65 cases(40%)were diagnosed as non-autoimmune diseases,in which 48 cases were singly positive with anticytoplasmic autoantibodies. The composition of the autoantibodies in the 2 groups was statistically significant(P<0.01). Conclusions The detection rate of the specific antibodies in anticytoplasmic autoantibodies is low. The specific antibodies of anticytoplasmic autoantibodies might beyond the ordinary autoantibodies. Anticytoplasmic autoantibodies could be detected both in autoimmune and non-autoimmune diseases. In autoimmune diseases,anticytoplasmic autoantibodies usually appear with antinuclear autoantibodies.
    The application significance of serum CYFRA21-1 change in therapeutic efficacy monitoring of advanced stage NSCLC

    Objective To investigate the significance of serum cytokeratin 19 fragment 21-1(CYFRA21-1) changes before and after chemotherapy in monitoring the therapeutic efficacy of advanced stage non small cell lung cancer(NSCLC).   Methods The serum CYFRA21-1 levels of 66 patients with advanced stage NSCLC before and after chemotherapy and 30 healthy controls were measured by chemiluminescence immunoassay,and the change differences of serum CYFRA21-1 in the patients with different therapeutic efficacy were analyzed. Results Compared to those in the controls,serum CYFRA21-1 levels were significantly higher than those in patients before the first-line chemotherapy(P<0.05). Furthermore,the serum CYFRA21-1 levels in patients with squamous cell lung carcinoma were higher than those with pulmonary adenocarcinoma(P<0.05). Compared to those before chemotherapy,serum CYFRA21-1 levels significantly decreased in release group after the first-line chemotherapy(P<0.05). However,compared to those before chemotherapy,serum CYFRA21-1 levels significantly decreased in invalid group and progress group(P<0.05). Conclusions Serum CYFRA21-1 is exactly valuable in monitoring the pathogenetic condition and judging the chemotherapy efficacy of the patients with lung cancer.

    Objective To study the changes and its clinical significance of T lymphocyte subsets and natural killer(NK) cells in peripheral blood of patients with histiocytic necrotizing lymphadenitis(HNL).   Methods The percentages of peripheral blood T lymphocyte subsets and NK cells of 51 HNL patients and 18 healthy controls were detected by flow cytometry(FCM). Results Compared with the healthy controls,the numbers of CD3+ cell,CD4+ cell,CD16+CD56+ cell and CD4+/CD8+ ratio decreased in HNL patients(P<0.05). After therapy,19 patients achieved complete remission,and 30 patients got improvement,and there was no statistical significance compared with healthy controls(P>0.05). Conclusions Cellular immune function is significantly abnormal in patients with HNL. The detections of T lymphocyte subsets and NK cells are clinically significant on the evaluation of therapeutic efficacy of HNL.

    Objective To investigate the interference of lipemia on automatic hematology analyzers.   Methods Normal fresh whole blood samples were determined for complete blood cell(CBC) count by automatic hematology analyzer,and the plasma were replaced by same volume of fat emulsion after centrifugal separation to make artificial lipemia [blood triglyceride(TG)>4.0 mmol/L]. These samples were tested for CBC by the same method before they were added lipid to estimate which parameters can be affected by lipemia.  Results When the TG>4.0 mmol/L,lipemia could make pseudo increase of hemoglobin(Hb),mean corpuscular hemoglobin(MCH) and mean corpuscular hemoglobin concentration(MCHC)(P<0.05). When the TG>14.45 mmol/L,fat particles can make pseudo increase of the white blood cell(WBC) count with statistical significance(P<0.05).  Conclusions When TG>4.0 mmol/L,Hb,MCH and MCHC need to be corrected. The higher TG concentration is,the greater interference of Hb,MCH and MCHC are. The lower Hb concentration is,the greater interference of TG is. When TG>14.45 mml/L,WBC needs to be corrected.

    Objective To investigate the influence of storage time of venous blood samples on the results of reticulocyte(RET) parameters.   Methods The blood samples of patients with and without anemia were detected respectively at different time after bleeding by Sysmex XE-2100 hematology analyzer,and the changes of RET parameters were analyzed. Results In non-anemia patients,only immature reticulocyte(IRF%),high fluorescence reticulocyte(HFR%),middle fluorescence reticulocyte(MFR%) and low fluorescence reticulocyte(LFR%) had significant changes after bleeding 72h,while there was no statistical significance for reticulocyte count(RET#) and reticulocyte percentage(RET %). However,in anemia patients,RET# and RET % reduced significantly after bleeding 72h(P<0.05),and IRF%,HFR%,MFR% and LFR% had significant changes after bleeding 24h. Conclusions Long time storage of venous blood samples has influence on the results of RET parameters. In anemia patients,the results can reflect the accuracy if the samples are detected within 24h after bleeding.
    The infection of hepatitis C virus and treponema pallidum among HIV single-infection couples in Queshan county

    Objective To understand the infection of hepatitis C virus(HCV) and treponema pallidum among human immunodeficiency virus(HIV) single-infection couples in Queshan county.   Methods A total of 362 HIV single-infection couples in Queshan county were enrolled as experiment group. The blood of 169 HIV-positive spouses and 339 HIV-negative spouses had been extracted. The anti-HCV and anti-treponema pallidum antibodies had been detected by enzyme-linked immunosorbent assay(ELISA). The 1 700 healthy subjects in local hospital were enrolled as healthy controls. Results The HCV infection rate of the HIV-positive spouses was 85.8%(145/169).It was significantly higher than that of HIV-negative spouses[34.8%(118/339),χ2=117.4,P<0.01]. The HCV infection rates of the 2 groups were higher than that of control group [4.6%(78/1 700),χ2=891.3,P<0.01]. The HCV infection rate of HIV single-infection couples was 91.1%. The difference of treponema pallidum incidence was not significant between experiment group and control group(χ2=1.764,P>0.05). Conclusions It is indicated that blood is the main route of HIV infection among HIV single-infection couples in Queshan county,and the HCV concurrent infection should be paid attention while preventing the pathophoresis of HCV.
    An investigation of extended spectrum beta-lactamases′ bacterium in faeces

    Objective To investigate the infection of extended spectrum beta-lactamases(ESBLs)′ bacterium in faeces from patients of Shanghai Pudong New Area People′s Hospital,which is helpful to control the nosocomial infection.   Methods ESBLs were screened and determined by flat test among healthy subjects and patients in the hospital. Results A total of 10.9% of healthy subjects′ faeces had ESBLs,and the positive rate was 76.8% of patients′ faeces with ESBLs in the hospital. There was statistical significance between them(P<0.01). Conclusions The positive rate of ESBLs′ bacterium in patients′ faeces in the hospital is much higher than that in healthy subjects′ faeces,and it is a latent risk factor of nosocomial infection.
    Comparison on the anti-interference abilities of immune turbidimetry and immune nephelometry for detecting specific proteins

    Objective To compare the anti-interference abilities of immune turbidimetry and immune nephelometry for detecting specific proteins [immunoglobulin G(IgG),immunoglobulin M(IgM) and C reactive protein(CRP)]through the interferers of free bilirubin(FBil),conjugated bilirubin(CBil),hemoglobin(Hb) and chyle(CH).   Methods A total of 6 mixed serum samples were prepared under 2 levels(high/low) of specific proteins(IgG:8-12 g/L,>16 g/L;IgM:1.0-1.5 g/L,>2.0 g/L;CRP:4-10 mg/L,>100mg/L). The 4 interferers were prepared into 5 different concentrations(FBil:656,1 312,1 968,2 624 and 3 280 μmol/L;CBil:688,1 376,2 064,2 752 and 3 440 μmol/L;Hb:9.9,19.8,29.8,39.7 and 49.6 μmol/L;CH:3 000,6 000,9 000,12 000 and 15 000 FTU) by rules(1∶5,2∶5,3∶5,4∶5 and 5∶5),and the interferers were added to the mixed serum. The concentration of the specific proteins was detected by immune turbidimetry and immune nephelometry respectively,and the interference rate was calculated. The anti-interference abilities of the 2 methods were calculated according to Clinical and Laboratory Standards Institute(CLSI) EP7-A standard. The average concentration of the specific proteins with different interferers ±5% of the control tubes at the same concentration was considered to have interference. Results By the immune nephelometry,the values to detect IgM(high level )and CRP( low level) with the interferer of Hb(9.9 and 29.8 μmol/L)and Hb(9.9,29.8 and 39.7 μmol/L) respectively,also the values to detect IgM(high level and low level ) with the interferer of CH(9 000,12 000 and 15 000 FTU)and CH(6 000,9 000,12 000 and 15 000 FTU) were all considered to have interference. However,by the immune turbidimetry,the values to detect IgG,IgM and CRP with the 4 interferers were considered to have no interference. Conclusions To 4 interferers(FBil,CBil,Hb and CH),the anti-interference ability of immune turbidimetry is better than that of immune nephelometry. The immune turbidimetry shows better accuracy to detect specimens with the interferers of lipidemia and haemolysis as large particles in the clinical application.

    Objective To investigate the comparability of the results of alpha fetoprotein(AFP) and carcinoembryonic antigen(CEA) measured by light initiated chemiluminescence(LiCA) and enzyme-enhanced chemiluminescence,and evaluate the clinical performances of LiCA AFP and CEA kits.   Methods According to the documents of National Committee for Clinical Laboratory Standards(NCCLS) and In Vitro Diagnostic Products Clinic Research Guideline,the analytical sensitivity,imprecision and linearity of LiCA AFP and CEA kits were evaluated,and 406 AFP serum samples and 442 CEA serum samples were detected by LiCA and enzyme-enhanced chemiluminescence respectively. The data were analyzed statistically. Results The analytical sensitivities of AFP and CEA by LiCA were 0.112 and 0.05 ng/mL. The within-run and between-run coefficients of variation(CV) of AFP and CEA by LiCA were 4.32%,4.56% and 5.89%,6.47% respectively. The linearities of AFP and CEA by LiCA were 2.6-950.2 and 1.2-862.6 ng/mL. The correlation coefficients(r) of 2 chemiluminescence systems for AFP and CEA were >0.99. Conclusions The clinical performances of LiCA AFP and CEA are satisfied to clinical measurement. There is a correlation between the 2 systems for AFP and CEA.
    Performance verification of the detection system of UF-500i automatic urinary sediment analyzer

    Objective To verify the main analysis performance of the UF-500i automatic urinary sediment analyzer,Sysmex,Japan.   Methods White blood cell(WBC),red blood cell(RBC),endothelial cell(EC) and bacterium(BACT) were selected to verify the performance of the equipment. The precision was evaluated by coefficient of variation. Carryover circumstance was evaluated by carryover rates. Linear range was evaluated by regression equation,and the comparison of different equipments was evaluated by relative deviation. Results The within-run and between-run precisions of WBC and RBC were <10%. The within-run and between-run precisions of EC and BACT were <30% and <20%,respectively. All the within-run and between-run precisions of selected testing items were within an acceptable range. The carryover rates of BACT were >0.05% without the requirements. All the other carryover rates of selected testing items were <0.1% within the requirements. The linear verification results got the regression equation of the theoretical and measured values after certain dilution,in which b value was within the range of 0.97-1.03(r>0.975). The relative deviation in different equipments was <1/2 total errors(TEa) provided by equipment production. Conclusions The precision,carryover rates,the comparison results in different equipments and the linearity of the selected testing items of the UF-500i automatic urinary sediment analyzer meet the requirements of the laboratories in clinical diagnosis and have the clinical application significance.

    Objective To research the sensitivity of colloidal gold immunochromatography assay kit in rapid antigen test of influenza A pandemic(H1N1) 2009 virus. Methods A total of 171 nasal-pharyngeal swabs were tested retrospectively by the colloidal gold immunochromatography assay kits,and the results determined by real-time fluorescence quantitative polymerase chain reaction(FQ-PCR) were as reference. Compared to the results of real-time FQ-PCR,the sensitivities of the 2 kits were then obtained at various courses of disease and virus RNA concentrations. Results The sensitivities of the 2 kits had significantly positive correlation with virus RNA concentrations(P<0.01),and reached both 73.3% when the virus RNA concentration was ≥108 copies/mL. The positive rates of the 2 kits showed significantly negative correlation with the courses of disease(P<0.01),and were 66.7% and 62.5% respectively between 1 to 2 d after the symptoms appeared. Theoretical study demonstrated that the sensitivities would be 77.3% and 74.6%. Conclusions The colloidal gold immunochromatography assay kit could provide early diagnostic efficacy of influenza A pandemic(H1N1) 2009 virus,and the feature is high virus concentration. It could be used in early diagnosis and screening test of massive specimens or field test for customs. Since the colloidal gold immunochromatography assay has some limitations,the results of the assay should be confirmed by PCR.
    Influence of TNF-α on the secretion of testosterone and the proliferation in theca interna cells

    Objective To study the influence of tumor necrosis factor-alpha(TNF-α) on the secretion of testosterone and the proliferation in theca interna cells,and to investigate the role of TNF-α in the pathogenesis of polycystic ovary syndrome(PCOS).   Methods Different concentrations of TNF-α(10,100 and 1 000 pg/mL) were treated with theca interna cells. The secretion of testosterone in theca interna cells were detected by radioimmunoassay(RIA),and the proliferation of theca interna cells were detected by CFDA SE kit and flow cytometry. Results The testosterone level had no difference among the groups(10,100 and 1 000 pg/mL)and the control group(0 pg/mL)at 24 h,48 h and 72 h in theca interna cells. After 72 h,the mean fluorescence intensities(MFI) in theca interna cells with TNF-α 100 pg/mL and 1 000 pg/mL were obviously lower than those in the control group(53.2±11.59 and 66.87±14.8,P=0.013;47.55±7.31 and 66.87±14.8,P=0.001). Conclusions Although TNF-α does not affect the secretion of testosterone,it can promote the proliferation of theca interna cells. This function may explain that PCOS usually accompanies thick ovarian tunica albuginea and proliferative theca cells.
