Application of the determination of reticulocyte hemoglobin concentration and hypochromic red blood cell percentage for the screening of iron deficiency anemia during pregnancy

Abstract: Objective To investigate the application significance of reticulocyte hemoglobin concentration(CHr) and hypochromic red blood cell percentage(Hypo%) for the screening of iron deficiency anemia(IDA)during pregnancy.   Methods The levels of red blood cell(RBC),hemoglobin(Hb),mean corpuscular volume(MCV),mean corpuscular hemoglobin(MCH),mean corpuscular hemoglobin concentration(MCHC),Hypo%,red cell volume distribution width(RDW),CHr and the percentage of reticulocyte(Ret%) in peripheral blood of pregnant women were determined by ADVIA2120 hematology analyzer. The levels of serum iron(SI) were measured simultaneously. Results The levels of CHr,RBC,Hb,MCV,MCH,MCHC,Hypo%,RDW and SI in IDA group had statistical significance compared with those in non-IDA group and normal group(P<0.05). Ret% had no statistical significance(P>0.05). The levels of CHr and Hypo% in IDA group were(26.8±3.4)pg and(7.5±3.7)%,which had statistical significance with those of non-IDA group [CHr(31.9±2.4)pg and Hypo%(1.6±1.6)%] and normal group [CHr(33.9±2.2) pg and Hypo%(0.7±0.8) %,P<0.01]. When the cut-off value was 29.0 pg,the sensitivity of CHr was 81.0%,the specificity was 93.0%,and the area under receiver operating characteristic(ROC) curve was 0.929. When the cut-off value was 6%,the sensitivity of Hypo% was 78.0%,the specificity was 95.0%,and the area under ROC curve was 0.933. Conclusions The determination of CHr and Hypo% can be used as sensitive and early indices in the diagnosis of IDA in pregnant women. The simultaneous detection of CHr and Hypo% can provide scientific reference for nutrition intaking in pregnant women.
