摘要:

非霍奇金淋巴瘤(NHL)是一种常见的恶性肿瘤,约占淋巴瘤的90%。当人类免疫缺陷病毒(HIV)感染时会增加NHL的发生率,因此该类疾病被称为获得性免疫缺陷综合征相关淋巴瘤(ARL)。目前,ARL的发生、发展尚未形成定论,多数学者认为患者由于HIV感染,致使CD4+T细胞减少,免疫功能下降,进而导致ARL发生。因此,最新的治疗方法一般从源头上进行抗逆转录病毒治疗(ART)及免疫功能重建,同时采取针对淋巴瘤的R-CHOP方案。ARL患者需要选择合适的个体化治疗方案,且其预后转归往往大相径庭。文章拟对近年来ARL发病机制、治疗及预后的研究进展进行综述,为临床诊疗提供新思路。

Abstract:

Non-Hodgkin's lymphoma(NHL),which accounts for 90% of lymphoma,is a common malignant tumor. When the body is infected with human immunodeficiency virus(HIV),the incidence rate of NHL will increase. This type of disease is called acquired immune deficiency syndrome-related lymphoma(ARL). At present,the occurrence and development of ARL has not been determined yet. Due to HIV infection,CD4+ T cells is reduced in patients with ARL,the immune status of patients is also disordered,and ARL occurs. Therefore,the new strategy of therapy focuses on antiviral therapy(ART) and the reconstruction of immune function with applying chemotherapy targeting to lymphoma by R-CHOP. Patients with ARL need to choose appropriate individualized tools,but the prognosis is often different. This review intends to summarize the studies on ARL,including mechanism,treatment and prognosis in recent years and provide new insights for clinical application.
