牙周炎患者病原菌感染状况及RANKL/OPG变化

Status of pathogenic bacteria and changes of RANKL/OPG in patients with periodontitis

摘要:

目的 探讨牙周炎患者的病原学状况及核因子κB受体活化因子配体(RANKL)/骨保护素(OPG)变化,为牙周炎的临床预防与治疗提供依据。方法 选取130例牙周炎患者作为观察组,另选取体检健康者50名作为对照组。比较2个组受检牙的牙周探诊深度(PD)、附着丧失(AL)、菌斑指数(PLI)和龈沟出血指数(SBI)等指标,分离鉴定病原菌,比较2个组龈沟液RANKL和OPG的差异,分析观察组的临床检查结果与RANKL、OPG的相关性。结果 观察组受检牙的PD、AL、PLI和SBI指标高于对照组(P<0.05)。观察组牙龈卟啉单胞菌、伴放线放线菌、生痰二氧化碳嗜纤维菌和齿蚀拟杆菌的检出率明显高于对照组(P<0.05)。观察组龈沟液中RANKL水平和RANKL/OPG高于对照组,OPG水平低于对照组(P<0.05)。观察组OPG水平与PD和AL呈负相关(P<0.05),RANKL水平和RANKL/OPG与PD和AL无相关性(P>0.05),RANKL水平、OPG水平及RANKL/OPG与PLI和SBI均无相关性(P>0.05)。结论 牙龈卟啉单胞菌、伴放线放线菌和生痰二氧化碳嗜纤维菌是牙周炎的主要致病菌,RANKL和OPG在牙周炎患者的牙槽骨组织破坏过程中发挥作用。

Abstract:

Objective To investigate the status of pathogenic bacteria and the changes of receptor activator of nuclear factor kappa-B ligand(RANKL)/osteoprotegerin(OPG) in patients with periodontitis ,and to provide a reference for the prevention and treatment of periodontitis.Methods A total of 130 patients with periodontitis were enrolled,and 50 healthy subjects were enrolled as control group. The periodontal probing depth(PD),attachment loss(AL),plaque index(PLI) and sulcus bleeding index(SBI) were compared between the 2 groups. The pathogenic bacteria were isolated and identified in the 2 groups. The RANKL and OPG levels in gingival crevicular fluid between the 2 groups were compared. The correlations with RANKL and OPG levels were evaluated.Results The PD,AL,PLI and SBI in periodontitis group were higher than those in control group(P<0.05). The detection rates of Porphyromonas gingivalis,Actinomycetes,Capnocytophaga sputigena and Bacteroides toothdecay were higher in periodontitis group than those in control group(P<0.05). The level of RANKL and RANKL/OPG in periodontitis group were higher than those in control group,and the OPG level was lower(P<0.05). There was a negative correlation of OPG level with PD and AL in periodontitis group(P<0.05). There was no correlation of RANKL level and RANKL/OPG with PD and AL(P>0.05). There was no correlation of RANKL and OPG levels and RANKL/OPG with PLI and SBI(P>0.05).Conclusions Porphyromonas gingivalis,Actinomycetes and Capnocytophaga sputigena are the main pathogenic bacteria for periodontitis. RANKL and OPG play roles in the destruction of alveolar bone tissue in patients with periodontitis.
