缺血性脑卒中患者血浆lncRNA PINK1-AS和miR-532-5p表达变化及其对睡眠质量和认知功能的影响

Expression changes of plasma lncRNA PINK1-AS and miR-532-5p in patients with ischemic stroke and their effects on sleep quality and cognitive function

摘要:

目的 探讨血浆长链非编码RNA(lncRNA)PINK1-AS、miR-532-5p表达与急性缺血性脑卒中(AIS)患者睡眠质量和认知功能的关系。 方法 选取2021年6月—2024年1月郑州大学附属郑州中心医院AIS患者129例(AIS组)、健康体检者124名(正常对照组)。收集所有研究对象的临床资料,并检测血浆lncRNA PINK1-AS和miR-532-5p表达。分别采用匹兹堡睡眠质量指数(PSQI)量表和蒙特利尔认知评估(MoCA)量表评估AIS患者发病3个月后的睡眠质量和认知功能,并将患者分别分为睡眠质量差组(61例)和睡眠质量好组(68例)、认知障碍组(40例)和认知正常组(89例)。采用Pearson相关分析评估各项指标与PSQI评分、MoCA评分的相关性。采用受试者工作特征(ROC)曲线评价血浆lncRNA PINK1-AS、miR-532-5p相对表达量判断AIS患者睡眠质量差和认知功能障碍的效能。 结果 AIS组血浆lncRNA PINK1-AS相对表达量高于正常对照组(P<0.001),血浆miR-532-5p相对表达量低于正常对照组(P<0.001)。睡眠质量差组血浆lncRNA PINK1-AS相对表达量高于睡眠质量好组(P<0.001),血浆miR-532-5p相对表达量低于睡眠质量好组(P<0.001),2个组之间各项临床资料差异均无统计学意义(P>0.05)。认知障碍组血浆lncRNA PINK1-AS相对表达量高于认知正常组(P<0.001),血浆miR-532-5p相对表达量低于认知正常组(P<0.001),2个组之间各项临床资料差异均无统计学意义(P>0.05)。AIS患者血浆lncRNA PINK1-AS表达与PSQI评分呈正相关(r=0.524,P<0.05),与MoCA评分呈负相关(r=-0.512,P<0.05);miR-532-5p表达与PSQI评分呈负相关(r=-0.528,P<0.05),与MoCA评分呈正相关(r=0.527,P<0.05)。血浆lncRNA PINK1-AS、miR-532-5p相对表达量单项检测和联合检测判断AIS患者睡眠质量差的AUC分别为0.809、0.857、0.928,判断AIS患者认知功能障碍的AUC分别为0.842、0.864、0.940。 结论 AIS患者血浆lncRNA PINK1-AS呈高表达,miR-532-5p呈低表达,且二者与患者睡眠质量差和认知功能障碍有关。

Abstract:

Objective To investigate the relationship between the expression of plasma long non-coding RNA(lncRNA)PINK1-AS and miR-532-5p and poor sleep quality and cognitive dysfunction in patients with acute ischemic stroke (AIS). Methods A total of 129 AIS patients(AIS group)and 124 healthy subjects(healthy control group)were enrolled from June 2021 to January 2024 at Zhengzhou Central Hospital of Zhengzhou University. The clinical data were collected,and the expressions of plasma lncRNA PINK1-AS and miR-532-5p were determined. The Pittsburgh sleep quality index(PSQI)and the Montreal cognitive assessment(MoCA)were used to evaluate sleep quality and cognitive function in AIS patients for 3 months after onset,and the subjects were classified into poor and good sleep quality groups (61 and 68 cases,respectively)and cognitive dysfunction and normal cognitive function groups(40 and 89 cases,respectively). Pearson correlation analysis was used to evaluate the correlation between various indicators. Receiver operating characteristic(ROC)curve was used to evaluate the efficacy of the relative expression levels of plasma lncRNA PINK1-AS and miR-532-5p in predicting poor sleep quality and cognitive dysfunction in AIS patients. Results The relative expression level of plasma lncRNA PINK1-AS in AIS group was higher than that in healthy control group(P<0.001),and the relative expression level of plasma miR-532-5p was lower than that in healthy control group(P<0.001). The relative expression level of plasma lncRNA PINK1-AS in poor sleep quality group was higher than that in good sleep quality group(P<0.001),and the relative expression level of plasma miR-532-5p was lower than that in good sleep quality group(P<0.001). There was no statistical significance in clinical data between the 2 groups(P>0.05). The relative expression level of plasma lncRNA PINK1-AS in cognitive dysfunction group was higher than that in normal cognitive function group(P<0.001),and the relative expression level of plasma miR-532-5p was lower than that in normal cognitive function group(P<0.001),with no statistical significance in clinical data between the 2 groups(P>0.05). The expression of plasma lncRNA PINK1-AS in AIS patients was positively correlated with PSQI score(r=0.524,P<0.05) and negatively correlated with MoCA score(r=-0.512,P<0.05). The expression of miR-532-5p was negatively correlated with PSQI score(r=-0.528,P<0.05)and positively correlated with MoCA score(r=0.527,P<0.05). The areas under curves (AUC) of the relative expression levels of plasma lncRNA PINK1-AS and miR-532-5p for predicting poor sleep quality in AIS patients were 0.809,0.857 and 0.928,respectively,and the AUC for predicting cognitive dysfunction in AIS patients were 0.842,0.864 and 0.940,respectively. Conclusions Plasma lncRNA PINK1-AS is highly expressed,miR-532-5p is lowly expressed in AIS patients,and they are related to poor sleep quality and cognitive dysfunction in AIS patients.
