红细胞分布宽度和单核细胞/淋巴细胞比值在强直性脊柱炎中的应用价值

摘要：

目的探讨红细胞分布宽度（RDW）和单核细胞/淋巴细胞比值（MLR）对强直性脊柱炎（AS）的诊断价值。方法收集吉林大学第一医院AS初诊患者131例、其他风湿性疾病患者137例和135名体检健康者资料,评估RDW及MLR与AS患者临床指标的相关性。结果 RDW、MLR均较正常对照者明显升高（P<0.05）,RDW和MLR在炎症相关指标高敏C反应蛋白（hs-CRP）、红细胞沉降率（ESR）界定的阳性患者和阴性患者间的差异有统计学意义（P<0.05）,MLR在IgG、IgA、IgM异常的患者和正常对照者间的差异有统计学意义（P<0.05）,且RDW与ESR、hs-CRP呈正相关（r_s值分别为0.277、0.225）,MLR与ESR、hs-CRP、IgG、IgA、补体3（C3）呈正相关（r_s值分别为0.358、0.399、0.214、0.183、0.241）。结论 RDW及MLR可作为一种简便、可行的生物学指标,在AS诊断及疾病观察中有一定价值。

Abstract:

Objective To evaluate the diagnostic value of red blood cell distribution width（RDW） and monocyte to lymphocyte ratio（MLR） in ankylosing spondylitis（AS）. Methods A total of 131 AS patients,137 other rheumatic diseases and 135 healthy subjects were enrolled from the First Hospital of Jilin University. The correlations of RDW and MLR with the clinical indexes of AS were evaluated. Results RDW and MLR were higher than those in healthy subjects（P<0.05）. There was statistical significance between RDW and MLR in the positive and negative patients defined by high-sensitivity C-reactive protein（hs-CRP） and erythrocyte sedimentation rate（ESR）（P<0.05）. There was statistical significance in MLR between IgG, IgA,IgM abnormal and normal patients（P<0.05）. RDW was positively correlated with ESR and hs-CRP,and the correlation coefficients（r） were 0.277 and 0.225, respectively. MLR was positively correlated with ESR,hs-CRP,IgG,IgA and complement 3（C3）, and the r were 0.358,0.399,0.214,0.183 and 0.241,respectively. Conclusions Both RDW and MLR may play roles for AS diagnosis and disease observation.
