    Glycosylation changes are the common characteristics of cancer cells. Some types of glycan structures are recognized as the markers of tumor development, but the current molecules cannot regularly meet the pressing needs of tumor early diagnosis, prognosis and metastasis judgment. In recent years, a remarkable progress has been made in the yield of carbohydrate antigens(CA) and their relative molecules in tumor diagnosis. In order to serve the clinical needs better, new tumor markers of CA and their relative molecules, together with multi-parameter analysis, should be established.

    Glycated hemoglobin A1c(HbA1c) is an important parameter for the diagnosis of diabetes mellitus and monitoring treatment efficiency. After HbA1c measurand being defined by the International Federation of Clinical Chemistry and Laboratory Medicine(IFCC),with efforts,the differences are decreasing gradually among clinical laboratories in the world. Although HbA1c manufacturers introduces their instruments' advantages and clinical significance mainly,instead of weaknesses,clinical laboratories should know instruments' characteristics and disadvantages. Instruments can be used into clinical determinations,only when the analysis performances have been verified and satisfy clinical application. Every laboratory should use 2 different methods,which can identify whether there is abnormal hemoglobin or not. Although HbA1c can be determined by immunological methods without interference from abnormal hemoglobin,the influence of abnormal hemoglobin has not been defined. In short,clinical laboratories should learn as much information as possible for patients.

    Objective To establish the reference intervals of serum free kappa(κ) light chain,free lambda(λ) light chain and their ratio in the population over 60 years old.Methods Epidemiological data(the histories of cardiovascular,hepatic,renal and diabetes mellitus diseases and pharmacohistory) were collected from 342 elder healthy subjects(227 cases of derivation cohort and 115 cases of validation cohort) in communities. Serum free κ light chain,free λ light chain,total κ light chain and total λ light chain were determined,and the ratio of free κ light chain to free λ light chain(free κ/λ ratio) was calculated. The results were classified and analyzed according to sex and age. The bilateral 2.5th and 97.5th percentiles were used to establish reference intervals,and the validation cohort's distribution of free κ/λ ratio in reference intervals was evaluated. Results There was statistical significance for hemoglobin,creatinine and calcium between males and females(P<0.05). There was no statistical significance for total κ light chain,total λ light chain,the ratio of total κ light chain to total λ light chain(total κ/λ ratio),free κ light chain,free λ light chain and free κ/λ ratio between males and females with different ages(P>0.05). There was no correlation of free κ/λ ratio with hemoglobin,creatinine and calcium(r=0.046,-0.071 and 0.054,P>0.05). According to the bilateral 2.5th and 97.5th percentiles,the reference intervals of free κ light chain,free λ light chain and free κ/λ ratio were 7.17-14.07 mg/L,23.04-60.59 mg/L and 0.15-0.46. There were 95.65%,95.65% and 96.52% subjects' free κ light chain,free λ light chain and free κ/λ ratio in validation cohort within the above 3 reference intervals. Conclusions The reference intervals of free κ light chain,free λ light chain and free κ/λ ratio in the population over 60 years old have been established,which can be used in the population over 60 years old.

    In the occurrence and development of tumors,the abnormal expression of sialyltransferase can regulate the biological characteristics of tumor cells. Abnormal sialylation can enhance the adhesion and metastasis ability of tumor cells,and can enhance tumor resistance to chemotherapy drugs. The abnormal expression of sialyltransferase is also related to the tissue and organ specificity of tumor metastasis. Sialic acid-modified glycoproteins with abnormal structures are of value in the early diagnosis and prognostic evaluation of tumors,which makes it possible to find new tumor markers. This review introduces the related mechanisms of sialylation affecting tumor occurrence and development in the recent years and the role of serum sialic acid markers in tumor diagnosis and prognosis.

    Objective To study whether a single s erum β-hCG determination in pregnancies ach ieved by assist ed reproductive technologies(ART) can accurately predict pregna ncy viability and inviable pregnancies, multiple gestation. Method Fo urty-nine consecutive succe ssful ART pregnancies between November 1998 and February 2000 were studied retrospectively. Fourty-six subjects had a serum β-hCG determinetion 14 day s after ET were analyzed. Results 36 of the 46(78.3%) were found to be viable. The mean β-hCG value of the nonviable was(89±24) U/L, the viable was(472±45)U/L,statistically significant differences were fou nd between thes e two groups, P<0.05. According to the cut off v alue of a single β-hCG>100 U/L in distinguishing viable from nonviable pregnancies achieved, the sensitivity was 97%, specificity 80%,the positive predictive value 95%,the negative p redictive value 89%. Ten of the 46 were multiple gestations, the mean β-hCG o f the multiple ge stations [(656±70) U/L] was significantly different from that o f the singleton pregnancies [(288±20)U/L].The probability of viable, and fu rthermore multiple p regnancies increase when the value of β-hCG goes up. Conclusion A single early serum β-hCG determinaton 14 alays after ET is clinically valuab le in predicting p regnancy outcome achieved by ART. It will help the gynecologists to take some m easurements in timely. Early reassuring tests may reduce an xiety of the couple.

    There are various methods for the determination of serum creatinine,and the specificities and anti-interference performance of methods are different. The determinations of serum creatinine in different clinical laboratories have biases. The interference of common drugs on creatinine determination has been drawn more and more attention. Many commonly-used drugs can lead to false results on creatinine determination. The interference directions vary from different methods of creatinine determination. The methods for creatinine determination and the interference of common drugs on creatinine determination are reviewed.

    Objective To establish a simple and rapid method for the determination of small dense low-density lipoprotein(sdLDL) by utilizing the principle of precipitation. Methods The final and most appropriate precipitation reagents were identified through comparing the separation effect of different types and different concentrations of precipitation reagents for separating serum lipoproteins. The precision, linear range and recovery rate of chemistry precipitation assay were also analyzed. Results Finally, 140 U/mL of heparin and 90 mmol/L of Mg2+ as the precipitation reagents were used to determine the concentrations of serum sdLDL cholesterol(sdLDL-C). The chemistry precipitation assay was linear at 0.14-1.44 mmol/L sdLDL-C, and the regression equation was Y=1.025X-0.197, r=0.988. The within-run coefficients of variation(CV) of high and low sdLDL-C concentration specimens were 2.83% and 3.19%, respectively, and the between-run CV were 5.49% and 6.13%. The recovery rate was 83.81%. Conclusions The chemistry precipitation assay for the determination of sdLDL has the advantages of less serum consumption, simple operation, low cost and so on. It is more suitable for clinical laboratory to determine serum sdLDL-C.

    The quality control indicators of clinical laboratories are essential in clinical laboratories as they enable the management to evaluate their performance objectively. As mentioned in the National Tertiary Public Hospitals Performance Evaluation Operational Manual (2022 edition),quality control indicators have been combined into performance appraisal system. Although 15 indicators from the Quality Control Indicators in Clinical Laboratory Service (2015) published by the National Health and Family Planning Commission,which are adopted in this manual,the interpretation of these indicators,the concepts of denominator and the numerator in the equations,as well as the datum acquisition methods are inconsistent,which leads to inaccurate results in performance evaluations between laboratories. This review makes the practice-based suggestions and opinions on interpreting and applying the 15 quality control indicators in clinical laboratories.

    Copeptin is the C-terminal peptide of the precursor molecule of arginine vasopressin(AVP),which can be released into the blood equimolar with AVP by equal molar,and the half-life of it is about 14 d,which is stable and easy to detect. Copeptin has low specificity and no organ specificity,but it has high positive predictive value and high sensitivity for acute myocardial infarction(AMI),stroke,chronic kidney disease and other diseases. This review focuses on the clinical application of copeptin in cardiovascular disease,liver disease,kidney disease,respiratory disease and stroke.

    Objective To investigate the role of human papillomavirus(HPV) DNA assay combined with thin prep liquid-based cytology test(TCT) in cervical lesion screening. Methods From July 2015 to April 2018,8 498 women undergoing routine cervical lesion screening were enrolled from Peking University International Hospital. HPV DNA assay and TCT were performed. The positive rate of high-risk HPV infection and the detection rate of TCT were calculated. The sensitivities and specificities of HPV DNA assay and TCT for the diagnosis of high-grade cervical lesions were evaluated. Results The positive rate of high-risk HPV infection was 12.2%. The sensitivity of TCT for the diagnosis of cervical lesions was 56.0%,and the specificity was 77.8%. The sensitivity of HPV DNA assay was 98.4%,and the specificity was 48.9%. The sensitivities of TCT and HPV DNA assay were 55.8% and 99.0% in patients with cervical intraepithelial neoplasia(CIN)≥2,respectively. Combined TCT with HPV DNA assay,the sensitivity was 90.1%,and the negative predictive value was 91.2%. Conclusions Using HPV DNA assay as primary screening method,TCT is performed or not according to the results of HPV DNA assay. This combined screening scheme is advantageous in the diagnosis of cervical lesions.

    Performance of real-time fluorescence PCR for the determination of CYP2 C19 polymorphisms

    Objective To evaluate the precision,accuracy and limit of detection of real-time fluorescence polymerase chain reaction(PCR) for cytochrome P450 2C19(CYP2C19) polymorphisms.Methods DNA samples with different CYP2C19 genotypes(*1/*1,*1/*2,*1/*3 and *1/*17) were determined for 10 times in 1 run for evaluating precision. The results of real-time fluorescence PCR and Sanger sequencing were used and compared for evaluating accuracy. The DNA samples with different genotypes(*1/*1,*1/*2,*1/*3 and *1/*17) were diluted in a 2-fold serial pattern and determined by real-time fluorescence PCR for evaluating limit of detection.Results Real-time fluorescence PCR showed low coefficient of variation(CV) of Ct values(<2.5%),a low limit of detection of 1.875 ng/μL and accuracy of 100%(20/20) compared to the results of Sanger sequencing,including 6 kinds of CYP2C19 genotypes in 20 cases(1 case of *1/*17,3 cases of *1/*1,5 cases of *1/*2,5 cases of *1/*3,4 cases of *2/*2 and 2 cases of *2/*3).Conclusions Real-time fluorescence PCR is precise,accurate and suitable for CYP2C19 polymorphisms.

    Construction of colon cancer prognostic model based on TCGA SpliceSeq alternative splicing events

    Objective To establish a genome-wide alternative splicing event risk model to predict the prognosis of colon adenocarcinoma(COAD). Methods COAD RNA-Seq data and clinical information were downloaded from the Cancer Genome Atlas(TCGA),and alternative splicing events were obtained from TCGA SpliceSeq database. Splice factors were downloaded from the SpliceAid 2 database. Univariate Cox regression analysis was employed to determine prognostic related alternative splicing events(PASE). Lasso regression was used to screenvariables. Multivariate Cox regression was performed to calculate risk score and constructmodels. Cytoscape Reactome FI plug-in was used to build an interactive network and find core nodes. GO and KEGG were implemented for gene function annotation and pathway analysis. Kaplan-Meier and receiver operating characteristic (ROC)curve were used for the evaluation of PASE risk model. PASE of SF and other genes were constructed to form prognosis-related interactive network. Results A total of 35 391 mRNA alternative splicing events occurred in 9 085 genes from 398 COAD patients,and 2 015 surviving PASEs occurred in 1 811 genes. In the risk model made by 8 PASE,patients were divided into high-risk group and low-risk group according to the optimal cut-off value of 0.919,and there were statistical difference between groups (P<0.001). The area under the ROC curve of the risk model was 0.860 (one year survival rate). In univariate Cox regression analysis,tumor invasion,lymph node metastasis,distant metastasis,clinical stages and risk model were significantly negatively correlated with overall survival time(P<0.001). After adjusting for multiple factors,the risk model still showed a significant negative correlation with the overall survival time of patients(P<0.001). In the risk model,there was no correlation between 8 PASE and mRNA expressions of their corresponding genes(P>0.05). Conclusion The effect of PASE on COAD prognosis is investigated by TCGA-COAD genome-wide analysis and a risk model has been constructed that can be used to predict clinical prognosis.

    Diagnostic and prognostic significance of AFP,CA199 and CEA levels in patients with hepatocellular carcinoma

    Objective To investigate the levels of alpha-fetoprotein (AFP),carcinoembryonic antigen (CEA) and carbohydrate antigen 199(CA199) in the carcinogenesis course of liver diseases and their prognostic significance. Methodse A total of 30 healthy subjects (healthy control group),38 patients with chronic hepatitis B,12 patients with hepatic cirrhosis,42 patients with primary hepatocellular carcinoma (PHC)and 12 patients with metastatic hepatocellular carcinoma were enrolled. The levels of AFP,CEA and CA199 were determined. The changes of AFP,CEA and CA199 were determined before operation for 1 d and after operation for 4 weeks and 8 weeks. Results The AFP levels of healthy control,chronic hepatitis B,hepatic cirrhosis,PHC and metastatic hepatocellular carcinoma groups were (11.2±59.87) μg/L,(27.9±17.83) μg/L,(68.91±19.87)μg/L,(668.18±260.74)μg/L and (414.64±170.33) μg/L. The levels of CEA were (2.72±1.21) μg/mL,(2.36±1.43)μg/mL,(4.09±1.67)μg/mL,(16.36±10.68) μg/mL and (31.91±17.44)μg/mL. The CA199 levels were (27.36±7.6)U/L,(18.19±6.6)U/L,(26.54±5.76) U/L,(59.09±18.67)U/L and (51.63±12.89) U/L. The positive rates for the combined determination of AFP,CEA and CA199 were 3.33%,15.79%,58.33%,88.09% and 84.62% in healthy control,chronic hepatitis B,hepatic cirrhosis,PHC and metastatic hepatocellular carcinoma groups. The AFP and CEA levels in recurrence group were (397.17±201.29)μg/Land (29.12±16.78) μg/mL(P<0.05),which were higher than those in non-recurrence group [(113.3±68.48) μg/L and (4.82±1.25) μg/mL]. Conclusions The combined determination of AFP,CEA and CA199 can improve the diagnosis rate of hepatocellular carcinoma,and it can be used for the diagnosis of PHC and metastatic hepatocellular carcinoma. AFP and CEA are of prognostic significance.

    China has made remarkable achievements in the prevention and control of human parasitic diseases,becoming the first country in the elimination of lymphatic filariasis. Malaria has been eliminated in China,which is approved by World Health Organization in 2021. Schistosomiasis in many provinces reaches the standard of transmission interruption or elimination. The infection rate of soil-transmitted nematode drops to a new historical low. However,the population of its infection is still tens of millions. Foodborne parasitic diseases,such as clonorchiasis sinensis,still pose a great threat to food safety in China. Although the vector-borne and zoonotic parasitic diseases have been controlled to some extent,the epidemic continues and relapses. The incidence of opportunistic parasitic diseases has increased in the population with immunodeficiency,and the cases of imported parasitic diseases have also increased. The review focuses on the results of parasitic diseases' prevention and control in China,understanding the epidemic status of important human parasitic diseases,analyzing the transmission risks and challenges of human parasitic diseases' prevention and control,so as to provide a reference for the health administrative department formulating the strategies and measures of parasitic diseases' prevention and control in the new period,and to provide epidemiological datum reference for the diagnosis and treatment of clinical parasitic diseases.

    Consistency of 2 systems for the determination of antinuclear antibody profile

    Objective To evaluate the consistency of commonly-used 2 systems for the determination of antinuclear antibody(ANA) profile,and to provide a reference for choosing methods for ANA profile determination. Methods Sera from 358 patients with systemic lupus erythematosus(SLE),147 patients with autoimmune disease and 47 healthy subjects were collected,and were determined for ANA profile by 2 systems,HUMAN(Germany) Diagnostics Ltd. and EUROIMMUN(China) Medical Diagnosis Ltd.. The calculated consistency and McNemar test were used to evaluate the consistency of 2 systems. Results The consistencies of anti-Smith(Sm) antibody,anti-ribosomal protein(RIB) antibody and anti-ribonucleoprotein(RNP) antibody were 79.9%,93.5% and 82.4%,with statistical significance(P<0.001). The consistencies of other 4 indices[anti-Sjogren syndrome A(SS-A) antibody,anti-Sjogren syndrome B(SS-B)antibody,anti-scleroderma 70(Scl-70) antibody and anti-Jo-1 antibody] were >90%,without statistical significance(P>0.05). Among 358 SLE patients,the positive rates of anti-Sm antibody were 48.6% by HUMAN and 27.1% by EUROIMMUN. Among 194 non-SLE patients,the specificities of anti-Sm antibody were 93.3% by HUMAN and 99.5% by EUROIMMUN.Conclusions The consistencies of HUMAN and EUROIMMUN in determining anti-Sm,anti-RIB and anti-RNP antibodies are low,while the other 4 indices show good consistencies. For the determination of anti-Sm antibody,HUMAN reveals a better determination rate and lower specificity than EUROIMMUN.

    Correlation of the poor prognosis of acute myeloid leukemia with FLT3- ITD and DNMT3 A mutations

    Objective To investigate the roles of FLT3-ITD and DNMT3A mutations in the prognosis of acute myeloid leukemia(AML) patients in intermediate-risk-cytogenetic-group. Methods A total of 132 de novo AML patients were enrolled from the Affiliated Hospital of Oregon Health and Science University from 2010 to 2016,and they were classified into favorable-risk-cytogenetic-group(24 cases),intermediate-risk-cytogenetic-group(79 cases) and adverse-risk-cytogenetic-group(27 cases). There were 2 cases with unknown cytogenetic information. Totally,42 genes being relevant to hematopoietic malignancies were determined by next generation sequencing(NGS). Results There were 91%(120/132) of AML cases with at least one mutation,and the pattern of co-mutations(≥2) accounted for 61%. Kaplan-Meier analysis demonstrated that the overall survival in FLT3-ITD mutation positive group for intermediate-risk-cytogenetic-group had statistical significance with that in FLT3-ITD mutation negative group(P<0.001). When DNMT3A was co-mutated,FLT3-ITD mutation positive group had a shorter overall survival compared to FLT3-ITD mutation negative group(P<0.001). In the present of DNMT3A wildtype,there was no statistical significance for overall survival between FLT3-ITD mutation positive and FLT3-ITD mutation negative groups(P>0.05). FLT3-ITD mutation positive with DNMT3A mutation group had a shorter overall survival compared to FLT3-ITD mutation negative group(P<0.001) and FLT3-ITD mutation positive with DNMT3A wildtype group(P=0.003). Conclusions In intermediate-risk-cytogenetic-group,the co-mutations of FLT3-ITD and DNMT3A have an adverse prognosis. The determinations of FLT3-ITDand DNMT3A mutations may be needed to refine prognostication and guide clinical management in AML.

    Diseases caused by Epstein-Barr virus (EBV) infection are increasing,such as organ transplantation-associated EBV infection. Epithelial cell source tumor is correlated with EBV infection,and it is characterized by recessive infection and is often ignored in clinic. With the development of determination methods for EBV infection,especially quantitative determination of EBV DNA,combined with the results of immunological determination,pathogens could be timely determined in early infection period. It is potential for the dynamic monitoring and prognosis evaluation of EBV-related tumors. This review focuses on the determination methods of EBV from home and abroad and the relationship between EBV-related determination and tumors.

    The accuracy of turbidimetric immunoassay is affected by a few interference factors,such as sample appearance,endogenous interference,hook effect and carry-over contamination. These interference factors may cause the results increasing or decreasing falsely and further affect clinical diagnosis and treatment monitoring. The laboratory staff should be familiar with common interference factors and mechanisms,communicate with doctors in time and identify,confirm and prevent possible interference,in order to reduce the occurrence of adverse events.

    Objective To evaluate the performance of improved uricase determination procedure and its uncertainty. Method According to A Candidate Reference Method for Uric Acid in Serum Optimization and Evaluation which was published by American Association for Clinical Chemistry (AACC),the accuracy was improved by standard solution gravimetric method. The precision was improved by multiple centrifugation. The accuracy and precision were evaluated,and the uncertainty was evaluated according to Guide to the Expression of Uncertainty in Measurement(GUM). Results The standard curve linear equation of uric acid was Y=0.005 74X+0.003 09(R2=0.999 99). The standard bias [coefficient of variation (CV)] was < 1.0%. The recovery ranged from 98.5% to 101.5%. The results of the International Federation of Clinical Chemistry and Laboratory Medicine (IFCC)external quality assessment scheme for reference laboratories in laboratory medicine(RELA) samples fell in the range of target value,with bias ±1.5% and relative standard uncertainty< 2.0%. Conclusions The improved uricase determination procedure is successfully established. It may provide an effective way for routine determination traceability of serum uric acid.
