γ-干扰素诱导蛋白-10 mRNA检测技术在肺外结核病诊断中的应用

Application of gamma-interferon inducible protein-10 mRNA determination technology in diagnosis of extrapulmonary tuberculosis

摘要:

目的 探讨基于γ-干扰素诱导蛋白-10(IP-10)mRNA的聚合酶链反应(PCR)荧光探针检测技术(即IP-10.TB)在肺外结核病诊断中的应用价值。方法 选取2023年3—10月南京中医药大学附属南京市中西医结合医院96例肺外结核病患者(结核病组)和51例非结核病变患者(非结核病组),分别进行IP-10.TB和γ-干扰素释放试验(IGRA),比较2种方法诊断肺外结核病的性能。计算IP-10.TB与Xpert MTB/RIF的符合率,比较IP-10.TB检测和IGRA不同类型肺外结核病阳性检出率的差异。结果 结核病组IP-10.TB阳性检出率(85.4%)高于非结核病组(0.0%)(χ2=98.518,P<0.001)。IP-10.TB、IGRA诊断肺外结核病的敏感性和特异性差异均无统计学意义(χ2值分别为3.200、0.033,P>0.05)。IP-10.TB与Xpert MTB/RIF的阳性符合率为96.3%,阴性符合率为50.0%,总符合率为89.1%。在淋巴结核和骨与关节结核这2种不同类型的肺外结核病中,IP-10.TB的阳性检出率差异无统计学意义(χ2=0.156,P=0.693)。结论 IP-10.TB诊断肺外结核病敏感性高、特异性好,可用于不同类型肺外结核病的快速诊断。

Abstract:

Objective To investigate the application value of gamma-interferon inducible protein-10(IP-10)mRNA based polymerase chain reation(PCR) fluorescent probe determination technology(IP-10.TB) in the diagnosis of extrapulmonary tuberculosis.Methods Totally,96 patients with extrapulmonary tuberculosis and 51 patients with nontuberculous lesions(non-tuberculous group) from March to October in 2023 at Nanjing Integrated Traditional Chinese and Western Medicine Hospital of Nanjing University of Traditional Chinese Medicine were enrolled for IP-10.TB determination and interferon gamma release assay(IGRA),and the performance was evaluated,as well as the consistency with Xpert MTB/RIF. Furthermore,the positive determination rates of IP-10.TB and IGRA in different types of extrapulmonary tuberculosis were evaluated.Results Compared with non-tuberculous group(0.0%),the positive determination rate of IP-10.TB in patients with extrapulmonary tuberculosis was higher(85.4%)(χ2=98.518,P<0.001). Compared with IGRA,there was no statistical significance in the sensitivity and specificity of IP-10.TB(χ2=3.200 and 0.033,P>0.05). The positive coincidence rate with Xpert MTB/RIF was 96.3%,the negative coincidence rate was 50.0%,and the total coincidence rate was 89.1%. There was no statistical significance in the positive determination rate of IP-10.TB in 2 different types of extrapulmonary tuberculosis,namely,lymphoid tuberculosis and bone and joint tuberculosis(χ2=0.156,P=0.693).Conclusions IP-10.TB is a method with high sensitivity and high specificity in the diagnosis of extrapulmonary tuberculosis,which can be used for rapid diagnosis of different types of extrapulmonary tuberculosis.
