氧化型高密度脂蛋白水平与2型糖尿病和肺结核共病的相关性

Correlation between oxidized high-density lipoprotein and comorbidity of T2DM and PTB

摘要:

目的 探讨氧化型高密度脂蛋白(ox-HDL)水平与2型糖尿病(T2DM)和肺结核(PTB)共病的相关性。方法 选取2022年6月—2023年6月深圳市慢性病防治中心门诊就诊的单纯高脂血症患者60例(单纯高脂血症组)、T2DM患者100例(T2DM组)、PTB患者100例(PTB组)和T2DM+PTB患者100例(T2DM+PTB组)。其中,T2DM组、PTB组和T2DM+PTB组均根据是否患有高脂血症分为血脂正常亚组和高脂血症亚组。另选取深圳市慢性病防治中心体检门诊的表观健康者60名,作为对照组。检测所有研究对象糖化血红蛋白A1c(HbA1c)、空腹胰岛素(INS)、空腹血糖(FSG)、总胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、载脂蛋白A1(apo A1)、载脂蛋白B(apo B)和ox-HDL。采用多元Logistic回归分析评价ox-HDL水平与PTB和T2DM+PTB的关系。结果 T2DM高脂血症亚组、T2DM+PTB高脂血症亚组、单纯高脂血症组和PTB高脂血症亚组ox-HDL水平均显著高于对照组(P<0.05)。T2DM高脂血症亚组ox-HDL水平显著高于单纯高脂血症组和PTB高脂血症亚组(P<0.05)。PTB高脂血症亚组、T2DM高脂血症亚组和T2DM+PTB高脂血症亚组ox-HDL/HDL-C比值和ox-HDL/LDL-C比值均显著高于对照组(P<0.05)。PTB高脂血症亚组、T2DM高脂血症亚组和T2DM+PTB高脂血症亚组TG均与ox-HDL呈正相关(r值分别为0.295、0.340、0.736,P<0.05)。多元Logistic回归分析显示,26 μg·L-1≤ox-HDL<52 μg·L-1是PTB患病的相对危险因素(P<0.05)。结论 升高的ox-HDL水平(26 μg·L-1≤ox-HDL<52 μg·L-1)或是T2DM和PTB共病的危险因素,26 μg·L-1≤ox-HDL<52 μg·L-1可作为一个有临床意义的病理水平去进一步评估。

Abstract:

Objective To investigate the correlation between oxidized high-density lipoprotein(ox-HDL) and the comorbidity of type 2 diabetes mellitus(T2DM) and pulmonary tuberculosis(PTB).Methods From June 2022 to June 2023,60 patients with simple hyperlipidemia,100 patients with T2DM,100 patients with PTB and 100 patients with T2DM+PTB were enrolled from the outpatient department of Shenzhen Center for Chronic Disease Control. Among them,T2DM group,PTB group and T2DM+PTB group were sub-classified into normal blood lipid subgroups and hyperlipidemia subgroups. In addition,60 apparently healthy subjects at Shenzhen Center for Chronic Disease Control were enrolled as control group. Glycated hemoglobin A1c(HbA1c),fasting insulin(INS),fasting serum glucose(FSG),total cholesterol(TC),triglyceride(TG),high-density lipoprotein cholesterol(HDL-C),low-density lipoprotein cholesterol(LDL-C),apolipoprotein A1(apo A1),apolipoprotein B(apo B) and ox-HDL were determined. Multiple Logistic regression analysis was used to evaluate the correlation between ox-HDL level and PTB,T2DM+PTB.Results The ox-HDL levels in T2DM hyperlipidemia subgroup,T2DM+PTB hyperlipidemia subgroup,simple hyperlipidemia and PTB hyperlipidemia subgroup were higher than that in control group(P<0.05). The ox-HDL level of T2DM hyperlipidemia subgroup was higher than that of simple hyperlipidemia group and PTB hyperlipidemia subgroup(P<0.05). The ox-HDL/HDL-C and ox-HDL/LDL-C in PTB,T2DM and T2DM+PTB hyperlipidemia subgroups were higher than those in control group(P<0.05). TG levels in PTB,T2DM and T2DM+PTB hyperlipidemia subgroups were positively correlated with ox-HDL(r=0.295,0.340 and 0.736,P<0.05). Multiple Logistic regression analysis showed that 26 μg·L-1≤ox-HDL<52 μg·L-1 was a risk factor for PTB(P<0.05).Conclusions Increased ox-HDL level(26 μg·L-1≤ox-HDL<52 μg·L-1) is a risk factor for T2DM and PTB comorbidity,and it should be further evaluated as a clinically significant pathological level.
