ITP患者血清IL-2、4、6、10及TNF-α、IFN-γ测定的临床意义分析

摘要: 目的 观察特发性血小板减少性紫癜(ITP)患者治疗前、后血清白细胞介素(IL)、肿瘤坏死因子-α(TNF-α)与γ-干扰素(IFN-γ)水平的变化。方法 采用酶联免疫吸附试验(ELISA)测定53例ITP患者治疗前、后及50名正常对照者血清IL-2、 IL-4、 IL-6、 IL-10、TNF-α、IFN-γ水平,并对检测结果进行分析。结果 53例ITP患者治疗前血清IL-2 IL-6、TNF-α及IFN-γ水平明显高于治疗后及正常对照组(P<0.05、P<0.01);血清IL-4、IL-10水平明显低于治疗后及正常对照组(P<0.01)。ITP患者治疗后除IL-6外,其余各项指标均恢复至正常对照组的水平(P>0.05)。结论 ITP作为一种自身免疫性疾病,存在多种细胞因子的异常,有效治疗后则多数细胞因子恢复至正常。因此,临床上对ITP患者应定期观察其血液中IL、TNF-α和IFN-γ水平变化,以判断其病情变化、治疗效果及其预后。

Abstract: Objective To evaluate the changes of serum interleukin(IL),tumor necrosis factor-alpha(TNF-α) and gamma-interferon(IFN-γ) in idiopathic thrombocytopenic purpura(ITP) patients before and after treatment. Methods The concentrations of serum IL-2,4,6,10,TNF-α and IFN-γ were respectively measured by enzyme-linked immunosorbent assay(ELISA) in 53 ITP patients before and after treatment and 50 healthy controls. The results were analyzed. Results The serum IL-2,IL-6,TNF-α and IFN-γ levels in 53 ITP patients before the treatment were significantly higher than those after the treatment and in the control group.(P<0.05,P<0.01). However,the serum IL-4 and IL-10 levels before the treatment were significantly lower than those after the treatment and in the control group(P<0.01). The serum IL-6 levels in 53 ITP patients after the treatment were higher than those in the control group,and the other parameters in 53 ITP patients after the treatment showed no significant difference with those in the control group(P>0.05). Conclusions ITP is characterized by the abnormal presence of multiple cytokines,as an autoimmune disease.The concentration of multiple cytokines can return to be normal after effective treatment.The serum concentrations of IL,TNF-α and IFN-γ in ITP patients should be regularly observed,in order to determine changes in their condition,treatment efficiency and prognosis.
