大黄对重症急性胰腺炎患者血清IL-10、IL-18影响的研究

Research on the influence of rude rhubarb on serum IL-10 and IL-18 levels in patients with severe acute pancreatitis

摘要: 目的 探讨大黄对重症急性胰腺炎(SAP)患者血清白细胞介素10(IL-10)、白细胞介素18(IL-18)的影响。方法 58例SAP患者随机分为A组(22例)、B组(36例)。A组只行综合治疗,B组在综合治疗基础上给予鼻空肠管注入大黄。采用酶联免疫吸附试验(ELISA)检测患者入院时 (0 d)、入院后7和14 d时血清IL-10、IL-18水平。以健康体检人群血清作为对照。结果 入院(0 d)时A、B组IL-18水平均显著高于对照组(P<0.01),IL-10水平也显著高于对照组(P<0.05),但IL-18、IL-10在A、B组之间差异无统计学意义(P>0.05)。治疗7 d时A、B组IL-18水平均较入院时下降,其中以B组下降最明显;A、B组之间IL-18水平差异有统计学意义(P<0.01);治疗7 d时A组IL-10水平与入院时无差异,但B组IL-10水平明显升高(P<0.01)。治疗14 d时A、B组IL-18水平均分别较治疗7 d时下降,但差异无统计学意义(P>0.05),而A、B组之间IL-18水平差异有统计学意义(P<0.01);治疗14 d时A、B组IL-10水平均分别较治疗7 d时升高,但差异无统计学意义(P>0.05),而A、B组之间IL-10水平差异仍有统计学意义(P<0.05)。在综合治疗基础上给予大黄治疗后,B组总有效率均明显高于A组(P<0.05)。相关分析显示,A、B组血清IL-18与血淀粉酶呈正相关(rA=0.55,P<0.05;rB=0.81,P<0.01),与尿淀粉酶也呈正相关(rA=0.51,P<0.05;rB=0.79,P<0.01);A、B组血清IL-10与血淀粉酶呈负相关(rA=-0.46,P<0.05;rB=-0.72,P<0.01),与尿淀粉酶也呈负相关(rA=-0.44,P<0.05;rB=-0.63,P<0.01)。结论 通过调节炎症介质IL-10、IL-18的表达,大黄能有效地逆转SAP病情。

Abstract: Objective To research the influence of rude rhubarb on serum interleukin 10 (IL-10) and interleukin 18 (IL-18) levels in patients with severe acute pancreatitis (SAP). Methods SAP patients were classified into group A(22 patients) and group B (36 patients) randomly.The patients in group A were given comprehensive treatment.On the basis of comprehensive treatment,the patients in group B were given rude rhubarb by nasojejunal feeding. The serum levels of IL-10 and IL-18 in the 2 groups were detected by enzyme-linked immunosorbert assay (ELISA) at different intervals (0 d, 7 d and 14 d). The sera of healthy subjects were as controls. Results On 0 d, the serum levels of IL-18 in A and B groups were higher than those in the control group (P<0.01), and the serum levels of IL-10 in A and B groups were also higher than those in the control group (P<0.05). However, the serum levels of IL-18 and IL-10 between A and B groups had no statistical significance (P>0.05). On 7 d,the serum levels of IL-18 in A and B groups decreased compared with those on 0 d, especially in group B, and there were statistical significances between A and B groups (P<0.01). On 7 d,the serum levels of IL-10 in A group didn′t change obviously compared with those on 0 d, but the serum levels of IL-10 in B group significantly increased compared with those on 0 d (P<0.01) .On 14 d, the serum levels of IL-18 in A and B groups decreased compared with those on 7 d with no statistical significance (P>0.05), but the levels of IL-18 between A and B groups showed significant differences(P<0.01). On 14 d, the serum levels of IL-10 in A and B groups increased compared with those on 7 d with no statistical significance (P>0.05), but the levels of IL-10 between A and B groups showed significant differences (P<0.05). The total effective rate in B group after administration of rude rhubarb was higher than that in A group (P<0.05). IL-18 and blood amylases were positively correlated (rA=0.55,P<0.05;rB=0.81,P<0.01), and IL-18 and urine amylases were positively correlated (rA=0.51,P<0.05;rB=0.79,P<0.01). There was negative correlation between IL-10 and blood amylases(rA=-0.46,P<0.05;rB=-0.72,P<0.01), and urine amylases(rA=-0.44,P<0.05;rB=-0.63,P<0.01)in A and B groups. Conclusions The administration of rude rhubarb can effectively reverse SAP by regulating the serum levels of IL-10 and IL-18.
