    Correlation between serum HIF-1α,CK-BB levels and the severity of neonatal hypoxic ischemic encephalopathy

    Objective To investigate the correlation between serum hypoxia inducible factor-1 alpha(HIF-1α) and creatine kinase isoenzyme-BB(CK-BB)and the severity of neonatal hypoxic ischemic encephalopathy(HIE). Methods A total of 230 neonates with HIE were enrolled and classified into 102 cases of mild,71 cases of moderate and 57 cases of severe according to the disease severity. Totally,200 healthy neonates were enrolled as healthy control group. Serum levels of HIF-1α and CK-BB were determined,and the 20 items of neonatal behavioral neurological assessment(NBNA) score were used to evaluate the severity of brain injury. Pearson correlation analysis was used to evaluate the correlation between serum HIF-1α,CK-BB levels and NBNA score. Results The levels of serum HIF-1α and CK-BB in neonatal HIE group were higher than those in healthy control group(P<0.001),and the NBNA score was lower than that in healthy control group(P<0.001). Serum HIF-1α and CK-BB levels were negatively correlated with NBNA scores(r=-0.701 and -0.683,P<0.05). In neonatal HIE group,with the increase of disease severity,the levels of serum HIF-1α and CK-BB were increased in turn(P<0.001),and the NBNA scores were decreased in turn(P<0.001). Conclusions Serum HIF-1α and CK-BB may be used as the indicators for evaluating the disease severity in HIE neonates.

    Changes of serum ProGRP,NSE and CYFRA21-1 levels among the patients with different stages of chronic kidney disease

    Objective To investigate the changes of serum gastrin-releasing peptide precursor(ProGRP),neuron-specific enolase(NSE) and cytokeratin 19 fragment(CYFRA21-1) among the patients with different stages of chronic kidney disease(CKD). Methods Totally,196 patients with impaired renal function were classified into 4 groups according to the criteria of renal function failure stages,including renal function compensation group [59 cases,creatinine(Cr) 133-177 μmol/mL],renal function decompensation group(49 cases,Cr 178-442 μmol/mL),renal failure group(46 cases,Cr 443-706 μmol/mL) and uremia group(42 cases,Cr≥707 μmol/mL). Totally,100 healthy subjects were enrolled as healthy control group. Serum ProGRP,NSE,CYFRA21-1 and Cr levels were determined. Results Serum ProGRP levels in healthy control group,renal function compensation group,renal function decompensation group,renal failure group and uremia group were all increased in turn(P<0.01). Serum NSE levels in renal function decompensation group,renal failure group and uremia group were all statistically significant(P<0.05),and all of them were higher than those in renal function compensation group and healthy control group(P<0.05),but there was no statistical significance between renal function compensation group and healthy control group(P>0.05). Serum CYFRA21-1 levels in all the groups with impaired renal function were higher than those in healthy control group(P<0.05),but there was no statistical significance among renal function decompensation group,renal failure group and uremia group(P>0.05). Among the 3 indexes,the level of serum ProGRP was increased the most significantly. Conclusions Renal function injury can lead to the increasing of ProGRP,NSE and CYFRA21-1 in different degrees. Attention should be paid to the effect of renal function on tumor markers in clinical practice.

    Relations of ZEB2 and E-Cad expressions in breast cancer tissues with prognosis

    Objective To analyze the expressions and prognostic values of E-box binding zinc finger protein 2(ZEB2) and epithelial-cadherin(E-Cad) in breast cancer tissues. Methods Tissue specimens of 80 patients with pathologically confirmed breast cancer and normal tissue specimens of adjacent tissues(≤3 cm from cancer tissues) were collected. Immunohistochemistry and real-time fluorescence quantitative polymerase chain reaction(RT-qPCR) were used to determine the expressions of ZEB2 and E-Cad proteins and their mRNA in breast cancer tissues and corresponding adjacent tissues. The medical history data of all the patients were collected. All the patients were followed up for 5 years,and the survival status was recorded. Kaplan-Meier survival curve was used to analyze the relations between the expressions of ZEB2 and E-Cad and the survival rate of breast cancer patients. Cox proportional hazard model was used to analyze the prognostic factors of 5-year survival rate of patients with breast cancer. Receiver operating characteristic(ROC) curve was used to analyze the efficacy of ZEB2 and E-Cad in breast cancer. Results The positive rate of ZEB2 in cancer tissues was higher than that in adjacent tissues(P<0.05),while the positive rate of E-Cad in cancer tissues was lower than that in adjacent tissues(P<0.05). There was a negative correlation between the expressions of ZEB2 and E-Cad in breast cancer tissues(r=-0.265,P=0.018). The relative expression of ZEB2 mRNA in cancer tissues was higher than that in adjacent tissues,while the relative expression of E-Cad mRNA in cancer tissues was lower than that in adjacent tissues. The expression of ZEB2 was related to tumor diameter,lymph node metastasis,TNM stage and human epidermal growth factor receptor-2(HER-2) expression. The expression of E-Cad was related to lymph node metastasis,TNM stage,estrogen receptor(ER) expression,progesterone receptor(PR) expression and HER-2 expression. The 5-year survival rates of ZEB2-negative and E-Cad-positive breast cancer patients was higher than that of ZEB2-positive and E-Cad-negative breast cancer patients,respectively(P<0.05). Cox proportional hazard model analysis showed that lymph node metastasis,TNM stage,ZEB2 expression and E-Cad expression were the prognostic factors for breast cancer patients [hazard ratio(HR)=3.47,6.49,2.35 and 0.15,respectively]. The results of ROC curve analysis showed that the areas under the curves for judging the 5-year survival of breast cancer by single and combined determinations of ZEB2 and E-Cad were 0.618,0.638 and 0.827,respectively. Conclusions ZEB2 and E-Cad proteins are abnormally expressed in breast cancer tissues,which may be used to evaluate the prognosis of breast cancer.

    Role of vascular endothelial growth factor in monitoring treatment outcome and evaluating the prognosis of patients with advanced melanoma

    Objective To investigate the application role of serum vascular endothelial growth factor(VEGF) in monitoring treatment outcome and predicting the prognosis of patients with melanoma. Methods A total of 182 healthy subjects and 49 melanoma patients were enrolled. Serum samples were collected for analyzing the levels of serum VEGF. Clinical data,including sex,age,clinical stage,chemotherapy,treatment outcome,primary site,ulcer,mutations of driver genes,were collected. All the patients were followed up for 24 months,and the progression-free survival time(PFS) and overall survival time(OS) were recorded. The PFS and OS were analyzed by Kaplan-Meier survival curve. The factors for PFS and OS in melanoma patients were analyzed by Cox regression analysis. Results Melanoma patients(48.98%)have a higher VEGF positive rate than healthy subjects(3.85%)(P<0.001). The melanoma patients were classified into 2 groups,VEGF normal group(25 cases) and VEGF increased group(24 cases). Increased VEGF level was related to poor treatment outcome(P=0.001),but it was not related to sex,age,tumor location,ulcer,clinical stage or driver gene mutation(P>0.05). For patients with increased VEGF level(3 months),PFS was shorter than that of patients with normal VEGF(9 months)(P=0.02),and OS was shorter without statistical significance(P>0.05). Patients with increased VEGF and advanced stage(Ⅳ)had increased risk of disease progression [hazard ratio(HR)=2.55 and 4.64,95% confidence intervals(CI) were 1.07-6.10 and 1.40-15.43]. Advanced stage(Ⅳ) was a risk factor for short OS in melanoma patients(HR=14.52,95%CI 2.67-79.08). Conclusions VEGF in serum may be a circulating biomarker to predict treatment outcome in patients with advanced melanoma.

    Correlation analysis of FGF-2,Lp-PLA2 and IGF-1 levels and disease severity in patients with diabetic foot

    Objective To investigate the changes of serum fibroblast growth factor 2(FGF-2),lipoprotein-related phospholipase A2(Lp-PLA2) and insulin-like growth factor-1(IGF-1) levels in patients with diabetic foot and the correlation with disease severity. Methods A total of 120 diabetic foot patients were enrolled as diabetic foot group. According to the Wagner classification,the diabetic foot patients were classified into grade 0-1 group(40 cases) and grade 2-3 group(48 cases) and grade 4-5 group(32 cases). Totally,60 type 2 diabetes mellitus(T2DM)patients were enrolled as T2DM group,and 60 healthy subjects were enrolled as control group. The changes of serum FGF-2,Lp-PLA2 and IGF-1 levels were evaluated,and Spearman method was used to analyze the correlation between the above indicators and the degree of diabetic foot disease. Results The levels of serum FGF-2 and Lp-PLA2 in control group,T2DM group and diabetic foot group were increased in turn(P<0.05),and the level of serum IGF-1 was decreased in turn(P<0.05). In diabetic foot patients,the levels of serum FGF-2 and Lp-PLA2 in grade 0-1 group,grade 2-3 group and grade 4-5 group were increased in turn(P<0.05),while serum IGF-1 level was decreased in turn(P<0.05). Serum FGF-2 and Lp-PLA2 levels were positively correlated with Wagner grade(r values were 0.652 and 0.665,P<0.05),and serum IGF-1 level was negatively correlated with Wagner grade(r=-0.643,P<0.05). Conclusions Serum FGF-2,Lp-PLA2 and IGF-1 levels are closely related to the disease severity of diabetic foot.

    Role of VZV DNA and IgG antibody determinations in aqueous humor of ARN patients in evaluation of antiviral efficacy

    Objective To investigate the dynamic changes of varicella-zoster virus(VZV) DNA load and IgG antibody level in aqueous humor of patients with acute retinal necrosis(ARN) who received antiviral treatment and the value in the evaluation of antiviral efficacy. Methods Totally,11 ARN patients receiving 8 weeks of intensive therapy were enrolled,and 100 μL of aqueous humor samples were collected once the patients were diagnosed and then on the 4th day,the 1st,2nd,3rd,4th and 8th weeks. VZV DNA load and VZV IgG antibody levels were determined. Spearman rank correlation analysis was used to evaluate the correlation between the logarithmic viral load and other indicators. Results The mean VZV DNA load in aqueous humor of 11 ARN patients before treatment was(6.97±0.57)lg(copies/mL). With the prolongation of antiviral treatment time,the VZV DNA load in aqueous humor showed an overall downward trend(P<0.000 1). After 8 weeks of treatment,10 of 11 patients(90.91%) had VZV DNA end up below the determination limit. The VZV IgG antibody level(S/CO value) in aqueous humor of 11 ARN patients was 4.84±1.72 before the treatment,which was positively correlated with the initial VZV DNA load(r=0.45,P<0.05). After receiving antiviral treatment,the level of VZV IgG antibody in aqueous humor of ARN patients showed a downward trend(P=0.001). Although the VZV IgG antibody level in aqueous humor of 11 ARN patients after 8 weeks of treatment was lower than that before treatment(P=0.001),it was still positive. Conclusions The 8-week intensive therapy effectively reduces the VZV load in the eyes of ARN patients. VZV DNA load determination can be used to monitor the progress of ARN disease and reflect the effect of antiviral treatment.

    Influence of the percentage of spermatozoa with normal morphology on in vitro fertilization embryo outcome after optimized semen treatment

    Objective To analyze the influence of the percentage of spermatozoa with normal morphology on in vitro fertilization(IVF)embryo outcome after optimized semen treatment,and to provide a reference for clinical treatment. Methods A total of 255 cases of IVF cycles were analyzed in which infertile was caused by women's fallopian tube disease,and oocytes were obtained ≥3. The semen optimized for IVF treatment was used. According to the related standards of World Health Organization,optimized semen were stained by modified Pap staining method and morphological analysis. According to the percentage of spermatozoa with normal morphology,they were classified into 2 groups,group A ≤19.03% and group B >19.03%. The fertilization rate,cleavage rate,available embryo rate,abnormal fertilization rate and high-quality embryo rate between group A and group B were compared. Results The fertilization rates of group A and group B were 77.8% and 79.6%,the cleavage rates were 95.4% and 96.0%,the available embryo rates were 82.0% and 85.4%,and the abnormal fertilization rates were 8.3% and 7.7%,respectively. The high-quality embryo rates were 71.4% and 71.6%,respectively. There was no statistical significance between the 2 groups in fertilization rate,cleavage rate,abnormal fertilization rate and high-quality embryo rate(P=0.247,0.461,0.549 and 0.958,respectively). The available embryo rate of group B was higher than that of group A(P=0.038). Conclusions The percentage of optimized spermatozoa with normal morphology can affect embryo outcome in IVF cycles. Early intervention is necessary to obtain good embryo outcome.

    Distribution of some common intestinal bacteria in elderly patients with acute cerebral infarction and its correlation with cognitive function

    Objective To observe the distribution of some common intestinal bacteria in elderly patients with acute cerebral infarction(ACI),and to analyze its correlation with cognitive function. Methods Totally,93 elderly patients with ACI admitted to the Third Affiliated Hospital of Gansu University of Chinese Medicine from April 2019 to April 2021 were enrolled as research subjects,and they were followed up for at least 6 months after thrombolytic therapy,which were classified into cognitive impairment group and normal cognitive group. The baseline data were collected,the distribution of some common intestinal bacteria in the 2 groups before treatment was determined,and the correlation between some common intestinal bacteria's distribution and cognitive function in elderly patients with ACI before treatment was analyzed. Results In 93 elderly patients with ACI at 6 months of follow-up after thrombolytic therapy,49 cases had cognitive impairment,the incidence rate was 52.69%. The numbers of Enterobacteriaceae and Enterococcus in cognitive impairment group were higher than those in normal cognitive group,and the numbers of Bifidobacterium,Lactobacillus and Peptococcus were lower than those in normal cognitive group(P<0.05). The results of Logistic regression analysis showed that a large number of Enterobacteriaceae and Enterococcus were risk factors for cognitive impairment in elderly patients with ACI [odds ratios(OR)were 1.777 and 1.689,P<0.05],a large number of Bifidobacterium,Lactobacillus and Peptococcus were protective factors for cognitive impairment in elderly patients with ACI(OR=0.213,0.242 and 0.409,P<0.05). Receiver operating characteristic(ROC) curve analysis showed that the areas under curves of the numbers of Enterobacteriaceae,Enterococcus,Bifidobacterium,Lactobacillus and Peptococcus before treatment in predicting cognitive impairment in elderly patients with ACI were 0.722,0.803,0.830,0.730 and 0.722,respectively,which had certain predictive value. Conclusions There is a correlation between cognitive function and the distribution of some common intestinal bacteria in elderly patients with ACI.

    Clinical application of metagenomic next-generation sequencing for determining pathogens in febrile neutropenic patients

    Objective To access the clinical role of metagenomic next-generation sequencing(mNGS) for determining pathogens in patients with febrile neutropenia(FN) after chemotherapy. Methods The data of 19 patients who were clinically diagnosed with FN after chemotherapy and were treated in Xi'an Central hospital from July 2019 to September 2020 were collected. The positive rates and spending times of mNGS,microculture and immunologic test were compared. The efficiency of mNGS,microculture and immunologic test [G test,GM test and Gram-negative bacterium lipopolysaccharide(GNBL)] was analyzed by receiver operating characteristic(ROC) curve. Results Totally,19 patients were enrolled,and 8 blood samples,7 throat swab samples,1 cerebrospinal fluid sample,2 hydrothorax and ascites samples and 1 urine sample were collected. The positive rates of mNGS,microculture and immunologic test were 68.4%,26.3% and 63.2%,respectively. Among the 13 positive samples in mNGS,there were 10 cases of Gram negative bacteria,4 cases of Gram positive bacteria,4 cases of fungi,3 cases of viruses and 1 case of mycoplasma. The spending times of mNGS,microculture and immunologic test were(47.0±13.2) h,(84.0±31.6) h and(24.8±16.4) h,and mNGS spent shorter time than microculture(P<0.05). The sensitivities of mNGS,microculture and immunologic test were 72.10%,30.70% and 70.9%,the specificities were 68.46%,82.94% and 30.70%,and the areas under curves were 0.806,0.688 and 0.550,respectively. There was statistical significance among the 3 methods(P<0.05). Conclusions The mNGS has good efficiency,and it is reliable than microculture and immunologic test,which has advantages in determining pathogens among FN patients after chemotherapy.

    Application evaluation of loop-mediated isothermal amplification in determining Clostridioides difficile in fecal samples

    Objective To evaluate a loop-mediated isothermal amplification(LAMP) for the rapid determination of Clostridioides difficile in fecal samples from patients. Methods Totally,132 clinical fecal samples were collected and determined by LAMP,bacterium culture and toxin Clostridioides difficile assay,respectively. The consistency of the 3 methods was evaluated. The specificity and sensitivity were evaluated. Results The sensitivity of LAMP for determing Clostridioides difficile reached 105 CFU/mL. The positive rate of LAMP(13.64%) was higher than that of bacterium culture(11.11%,χ2=18.74,P<0.05) and that of toxin Clostridioides difficile assay(9.09%,χ2=35.94,P<0.05). The LAMP showed all negative for the 11 bacteria and 2 fungi determined by bacterium culture. Conclusions Compared with bacterium culture and toxin Clostridioides difficile assay,the LAMP surpasses in good specificity and short reaction time. It can be regarded as a rapid determination method for Clostridioides difficile.

    Clinical application of gene chip technology for determining respiratory pathogens rapialy in pulmonary infection patients after renal transplantation

    Objective To evaluate the clinical application of gene chip technology for determining respiratory pathogens in the diagnosis of pulmonary infection after renal transplantation. Methods A total of 290 respiratory tract specimens(sputum,broncho-alveolar lavage fluid,throat swab) were collected from the post-renal transplantation patients. The pathogens were determined and cultured by gene chip and traditional bacterial culture. The results of 2 methods were analyzed statistically. Results A total of 290 patients after renal transplantation were enrolled,including 149 pulmonary infection patients. The pathogens determined by gene chip were found in 55 samples,and the sensitivity was 65.48%(55/84). The traditional bacterial culture only had 50 positive results,and the sensitivity was 64.00%(32/50). The 141 patients who were without pulmonary infection determined by gene chip had 112 negative results,and the specificity was 54.37%(112/206). The traditional bacterial culture had 123 negative results,and the specificity was 51.25%(123/240). Gene chip technology has statistical significance in clinical diagnosis of pulmonary infection(χ2=8.62,P<0.01). Conclusions Compared with the traditional bacterial culture method,gene chip has better sensitivity in the diagnosis of pulmonary infection after renal transplantation,and its specificity should be improved.

    N6-methyladenosine(m6A) is a dynamic reversible modification with the highest concentration of mRNA in mammalian cells. It is composed of methylases [methyltransferase like(METTL)3,METTL14,recombinant Wilms tumor 1 associated protein(WTAP)],demethylases [fat-mass and obesity-associated protein(FTO),alk B homolog(ALKBH)5],methyl-binding proteins [YTH N6-methyladenosine RNA binding protein 1(YTHDF1),YTH domain-containing(YTHDC)1,YTHDC2] and so on. The m6A methylation is related to the growth and development of human body and the occurrence and development of many diseases. In this review,the molecular mechanism,biological function,detection methods and the latest research progress of m6A are described,and the clinical application significance of m6A detection is prospected.

    Gastric cancer is one of the most common malignant tumors in the world. Due to its high heterogeneity,the 5-year survival rate is <30%. In the future,it is needed to find new and reliable biological indicators to improve the early diagnosis rate and the prediction accuracy of recurrence and metastasis of gastric cancer,so as to guide the treatment and improve the prognosis. Circulating tumor DNA(ctDNA) is a kind of free DNA released from apoptotic and necrotic tumor cells,which carries the mutation information related to tumor. The ctDNA was usually determined by liquid biopsies. It can be used as a biomarker for the dynamic monitoring of gastric cancer. It is safe,minimally invasive,convenient and repeatable. It has clinical significance in early diagnosis,disease monitoring and individualized treatment. This review expounds the biological characteristics,detection methods and the role of ctDNA in the diagnosis,treatment and prognosis evaluation of gastric cancer,and it discusses the clinical application value of ctDNA in gastric cancer.

    Research progress on the relationship between interleukin gene polymorphisms and gastric cancer

    Interleukins(IL) are common inflammatory cytokines that can drive a variety of cellular processes including metastasis,infiltration,growth and angiogenesis of tumor cells by paracrine and autocrine means. Gastric cancer is a type of gastrointestinal malignancy that is difficult to diagnose at an early stage,and its development is related to a variety of factors. Recent genetic studies have revealed that many single nucleotide polymorphism(SNP) loci in cytokine-encoding genes are related with the development of various cancers and diseases,suggesting that IL gene polymorphisms may play roles in the occurrence,development and metastasis of gastric cancer,and it is associated with the risk and susceptibility of gastric cancer development. Theoretically,the study of IL gene polymorphisms can be used to find gastric cancer-related diagnostic and treatment targets and elucidate the regulatory mechanisms of IL genes in gastric cancer,providing a new reference for its clinical diagnosis and treatment. This review focuses on the correlation of IL gene polymorphisms of gastric cancer and its research progress.

    Bacterial integrons have unique structures that can capture and express foreign genes,and they can be transmitted horizontally between bacteria through transposons and plasmids. The emergence of multi-drug-resistant bacteria is a challenge for clinical treatment,and many drug-resistant bacteria carry integrons. Therefore,the early determination of integrons is important for the early determination of bacterial drug resistance. At present,the methods of bacterial integron determination are mainly to determine the integrase of integron and to determine gene cassettes. This review introduces several bacterial integron determination methods,including the determination methods based on polymerase chain reaction(PCR),loop-mediated isothermal amplification(LAMP),metagenomic next generation sequencing(mNGS),gene chip technology. The principle and characteristics of various determination methods are introduced,and the advantages and disadvantages of methods are compared. This review provides a reference for finding a better determination method for clinical use,so as to grasp the situation of bacterial integron carrying in real time and respond to bacterial drug resistance as soon as possible.
