类风湿性关节炎患者早期外周血单个核细胞Toll样受体2、4表达及意义

Study on the expression and significance of Toll-like receptor 2 and 4 of peripheral blood mononuclear cells in patients with early-stage rheumatoid arthritis

摘要: 目的 探讨类风湿性关节炎(RA)早期患者外周血单个核细胞(PBMC)Toll样受体(TLR)2、TLR4对其配体双糖链蛋白多糖( BGN)、脂多糖(LPS)的刺激反应性,阐明PBMC TLR2、TLR4在RA疾病早期中的作用。方法 采用流式细胞术、实时荧光定量逆转录(RT)-聚合酶链反应(PCR)、酶联免疫吸附试验(ELISA)分别检测BGN和LPS刺激前后,RA组和健康对照组外周血CD14+单核细胞TLR4+细胞频率、PBMC TLR2 mRNA和TLR4 mRNA及上清液白细胞介素6(IL-6)和肿瘤坏死因子α(TNFα)的含量变化。结果 RA早期患者TLR2 mRNA明显升高、TLR4 mRNA下降(P<0.01);经LPS刺激后,RA患者TLR4 mRNA升高3.50倍,而健康对照组下降到0.11倍;LPS 和 BGN促进了各组PBMC产生IL-6、TNFα,但RA早期患者组上升的倍数明显高于健康对照组。结论 PBMC TLR2、TLR4参与早期RA的发生、发展。

Abstract: Objective To investigate the effects of Toll-like receptor(TLR)2 and TLR4 of peripheral blood mononuclear cells(PBMC) in patients with early-stage rheumatoid arthritis (RA) and the responsiveness of TLR4 to lipopolysaccharide(LPS) and TLR2 to biglycan(BGN).  Methods The expressions of TLR4+/CD14+ monocytes,TLR2 mRNA, TLR4 mRNA and cytokines of interleukin-6(IL-6) and tumor necrosis factor alpha(TNFα) in supernatants were analyzed by flow cytometry,real-time fluorescence quantitation reverse transcription(RT)- polymerase chain reaction(PCR) and enzyme-linked immunosorbent assay(ELISA) respectively in PBMC before and after stimulation.  Results The TLR2 mRNA in PBMC of RA patients were significantly higher than those of healthy controls(P<0.01). The TLR4 mRNA of RA patients were significantly lower than those of healthy control. LPS increased TLR4 mRNA of RA patients(3.50-fold) and decreased TLR4 mRNA of healthy controls(0.11-fold). LPS and BGN increased cytokines′ production in all groups,while the increased folds of IL-6 and TNFα in PBMC from RA patients were significantly higher than the cells from healthy controls.  Conclusions TLR2 and TLR4 of PBMC display important roles in the development of early-stage RA.
