血清sd-LDL-C和AIP与冠心病严重程度的相关性

Correlation between serum sd-LDL-C and AIP and the severity of coronary heart disease

摘要:

目的 探讨小而密低密度脂蛋白胆固醇(sd-LDL-C)和血浆致动脉粥样硬化指数(AIP)与冠心病(CHD)严重程度的相关性。方法 选取2023年6月1日—2024年1月31日保定市第一中心医院CHD患者139例(CHD组)。根据冠状动脉病变程度,将CHD组分为轻度组(Gensini评分<31分,53例)、中度组(Gensini评分31~60分,50例)、重度组(Gensini评分>60分,36例)。另选取同期存在胸闷、胸痛等心前区不适症状,冠状动脉造影(CAG)检查结果未见明显异常的患者36例作为对照组。检测CHD组与对照组的血清sd-LDL-C、总胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、载脂蛋白A1(apo A1)、载脂蛋白B(apo B),并计算AIP。采用多因素Logistic回归分析评价CHD发生的影响因素。采用Spearman秩相关分析评价sd-LDL-C、AIP与CHD患者冠状动脉病变程度的相关性。采用受试者工作特征(ROC)曲线评价各项指标单项检测和联合检测诊断CHD的效能。结果 CHD组HDL-C、apo A1低于对照组,TC、TG、LDL-C、apo B、sd-LDL-C、AIP高于对照组(P<0.05)。TC、TG、HDL-C、LDL-C、apo A1、apo B、sd-LDL-C、AIP均是CHD发生的危险因素(P<0.05)。轻度组、中度组、重度组sd-LDL-C、AIP依次升高(P<0.05)。sd-LDL-C与CHD患者冠状动脉病变程度呈正相关,等级相关系数(rs)为0.602(P<0.001);AIP与CHD患者冠状动脉病变程度亦呈正相关,rs值为0.261(P=0.012)。AIP、sd-LDL-C诊断CHD的曲线下面积(AUC)分别为0.839、0.783(P<0.05);sd-LDL-C、AIP联合检测的AUC为0.857,高于单项检测(P<0.05)。结论 CHD患者sd-LDL-C、AIP显著升高,且与冠状动脉病变程度密切相关,在CHD辅助诊断和病情严重程度评估中均有一定的价值。

Abstract:

Objective To investigate the correlation between small and dense low-density lipoprotein cholesterol(sd-LDL-C) and atherogenic index of plasma(AIP) and the severity of coronary heart disease(CHD). Methods Totally,139 patients with CHD from Baoding First Central Hospital from June 1,2023 to January 31,2024 were enrolled as CHD group. According to the severity of coronary artery lesions,they were classified into mild group(Gensini score <31 points,53 cases),moderate group(Gensini score 31-60 points,50 cases) and severe group(Gensini score >60 points,36 cases). Another 36 patients with chest tightness,chest pain and other discomfort symptoms in the precordial area,but with no obvious abnormalities in coronary angiography(CAG),were enrolled as control group. Serum sd-LDL-C,total cholesterol(TC),triglyceride(TG),high-density lipoprotein cholesterol(HDL-C),low-density lipoprotein cholesterol(LDL-C),apolipoprotein A1(apo A1) and apolipoprotein B(apo B) were determined,and AIP was calculated. Multivariate Logistic regression analysis was used to evaluate the influencing factors of CHD. Spearman rank correlation analysis was used to evaluate the correlation between sd-LDL-C,AIP and the severity of coronary artery lesions in CHD. Receiver operating characteristic(ROC) curve was used to evaluate the diagnostic value of each single and combined determinations for CHD. Results HDL-C and apo A1 in CHD group were lower than those in control group,while TC,TG,LDL-C,apo B,sd-LDL-C and AIP were higher in CHD group than in control group(P<0.05). TC,TG,HDL-C,LDL-C,apo A1,apo B,sd-LDL-C and AIP were risk factors for CHD(P<0.05). The sd-LDL-C and AIP were increased successively in the mild group,moderate group and severe group with statistical significance(P<0.05). The sd-LDL-C was positively correlated with the severity of coronary artery lesions,with a rank correlation coefficient (rs) of 0.602(P<0.001). AIP was positively correlated with the severity of coronary artery lesions,with the rs of 0.261(P=0.012). The areas under curves(AUC) of AIP and sd-LDL-C for predicting CHD were 0.839 and 0.783,respectively(P<0.05). The AUC of combined determination of sd-LDL-C and AIP was 0.857,which was higher than those of single determinations(P<0.05). Conclusions The sd-LDL-C and AIP are elevated in CHD patients,and are positively correlated with the severity of coronary artery lesions in CHD,which have certain value in diagnosing CHD auxiliarily and evaluating the severity of the disease.
