多重PCR系统快速检测儿童血流感染7种常见病原菌

摘要:

目的 建立快速检测儿童血流感染常见病原菌的多重聚合酶链反应(PCR)系统。方法 在16S rDNA-PCR结合测序鉴定血流感染病原菌的基础上筛选出7种儿童血流感染常见病原菌,针对这些病原菌设计特异性引物组合成多重PCR系统,用临床常见的15种细菌、4种真菌及人类细胞检测该系统的检测性能,并将该系统与血培养法及16S rDNA-PCR进行比较。结果 180例血液样本中血培养法阳性14例,阳性率为7.8%;16S rDNA-PCR阳性28例,阳性率为15.6%;多重PCR系统阳性25例,阳性率为 13.9%。多重PCR系统病原菌检出率高于血培养法(P<0.05),与16S rDNA-PCR比较,差异无统计学意义(P>0.05)。结论 所建立的多重PCR系统能够快速、准确地检测引起儿童血流感染的7种常见病原菌,比常规血培养法提早3~5 d将病原菌鉴定到种,对临床及时合理用药具有指导意义。

Abstract:

Objective To establish a multiplex polymerase chain reaction (PCR) system to detect common bacteria in children with bloodstream infection. Methods A total of 7 kinds of common bacteria were screened out based on the results of 16S rDNA-PCR and sequencing,and then the special primers were designed to establish a multiplex PCR system. The determination performance was evaluated by using 15 bacteria,4 fungi and human cells. The results of multiplex PCR system were compared with those of blood culture and 16S rDNA-PCR. Results The positive rate of blood culture (14 cases), 16S rDNA-PCR (28 cases) and multiplex PCR system (25 cases) were 7.8%,15.6% and 13.9%, respectively. This multiplex PCR system had a higher detection rate than blood culture(P<0.05) ,and there was no statistical significance compared with 16S rDNA-PCR(P>0.05). Conclusions The established multiplex PCR system exhibits high speed and accuracy for the detection of 7 kinds of common bacteria,which is earlier for 3-5 d than blood culture in identifying bacterium species,and it can guide clinicians for rational antibacterial agent usage in time.
