Correlation between the markers of bone metabolism and bone mineral density in elderly patients with type 2 diabetes mellitus

Abstract:

Objective To evaluate the correlation between the markers of bone metabolism and bone mineral density in patients with type 2 diabetes mellitus(T2DM).Methods The bone mineral densities of lumbar spine and hip bone were measured by dual-energy X-ray absorptiometry in 200 elderly patients with T2DM. According to the bone mineral densities,patients were classified into 3 groups,including normal group(75 cases),osteopenia group(72 cases) and osteoporosis group(53 cases). The height,weight and body mass index(BMI) for all patients were recorded. The markers of bone metabolism,including total type Ⅰ collagen N-terminal peptide(TPⅠNP),type Ⅰ collagen carboxyl-terminal peptide(β-CTX),bone gamma-carboxyglutamic-acid-containing protein(BGP) and 25-hydroxyvitamin D [25(OH)D],were determined. The levels of fasting blood glucose(FBG),glycated hemoglobin A1c(HbA1c),fasting C peptide,parathyroid hormone(PTH),serum calcium,24 h urinary protein(UP) were determined by electro-chemiluminescence.Results Compared with normal group,the levels of BMI,fasting C peptide,25(OH)D,BGP and TPⅠNP were decreased(P<0.05),while the levels of β-CTX,24 h UP,PTH and HbA1c were increased in osteopenia and osteoporosis groups(P<0.05). For age,diabetes mellitus duration,FBG and calcium levels,there was no statistical significance among the 3 groups(P>0.05). Moreover,bone mineral densities were positively correlated with the levels of BGP,TPⅠNP and 25(OH)D (r=0.273,0.269 and 0.223,P<0.01,0.05 and 0.01). The level of β-CTX showed a negative correlation with bone mineral densities(r= -0.246,P<0.01). Conclusions The markers of bone metabolism are related to bone mineral density in patients with T2DM,who easily merge with osteopenia,even osteoporosis.
