脂蛋白（a）室间质量评价反馈结果分析

摘要：

目的运用不同方法分析惠州市第一人民医院检验科脂蛋白（a）项目参加2014—2016年上半年国家卫生计生委临床检验中心室间质量评价（EQA）的反馈结果,探讨更有效的分析方法,减少该项目可能存在的误差。方法使用偏差、能力验证（PT）得分、Z比分数和多规则质量控制方案对惠州市第一人民医院检验科2014—2016年脂蛋白（a）项目5个批次25个EQA成绩进行统计分析。结果 2014—2016年脂蛋白（a）项目检测结果偏差符合国家卫生计生委临床检验中心要求,PT得分均为100分,|Z|<1.0,结果为满意。多规则质量控制方案分析显示2014年第1次、第2次以及2015年第2次检测结果都在均值一侧,且至少有1个样本结果偏差超过允许总误差（TEa）的50%,3次EQA结果均违背5+1_50%TEa规则,提示存在系统误差。结论惠州市第一人民医院检验科脂蛋白（a）项目EQA成绩良好,达到了国家卫生计生委临床检验中心的要求,但仍有进一步提高检测质量的空间。与其他EQA评价方法相比,多规则质量控制方案对误差检出率具有更高的敏感性。

Abstract:

Objective To analyze the results of lipoprotein（a） [Lp（a）] determination with different methods in Huizhou First People's Hospital Department of Clinical Laboratory participating external quality assessment （EQA） of the National Center for Clinical Laboratories from 2014 to 2016,and to choose effective methods and reduce the possibility of bias. Methodse A total of 25 EQA results of Lp（a）determination in 5 batches from 2014 to 2016 were statistically analyzed by bias,proficiency testing （PT）,Z score and quality control multi-rules. Results The results of bias met the requirements of EQA. PT scores were all 100,and |Z| was <1.0,which meant satisfaction. Quality control multi-rules showed the 1st and 2nd EQA in 2014 and the 2nd EQA in 2015 had at least 1 sample's results exceeding 50% allowable total error （TEa）,and all of them were against the rules of 5+1_50%TEa,which meant that systematic error was existing. Conclusions The results of Lp（a）determination are satisfactory and meet the requirements of the National Center for Clinical Laboratories,which can be further improved. The sensitivity of quality control multi-rules is higher than other indices of EQA.
