Abstract:

Objective To analyze the relationship between the levels of serum fatty acid binding protein 4(FABP4),paraoxonase-1(PON-1) and lipocalin 2(LCN2) in patients with acute myeloid leukemia(AML) and pathological characteristics, disease recurrence. Methods A total of 114 patients with AML(AML group) and 116 patients with acute lymphoblastic leukemia(ALL) diagnosed in Nanyang Central Hospital from January 2023 to January 2024,as well as 116 healthy subjects(healthy control group),were enrolled. According to the results of recurrence assessment, the AML group was further classified into remission group(72 cases) and recurrence group(42 cases). Serum levels of FABP4,PON-1 and LCN2 were determined using enzyme-linked immunosorbent assay, and the differences in various indicators among the groups were compared. Multivariate Logistic regression analysis was used to evaluate the risk factors for recurrence in AML patients after treatment. Receiver operating characteristic(ROC) curve was used to evaluate the efficacy of serum FABP4,PON-1 and LCN2 levels in predicting recurrence in AML patients after treatment. Results Serum FABP4,LCN2 levels were progressively higher in healthy control group,ALL group and AML group, while PON-1 levels were progressively lower(P<0.05). There was statistical significance in the prognostic risk stratification between high-expression groups of FABP4,PON-1 and LCN2(> mean values) and low-expression groups of FABP4,PON-1 and LCN2(≤mean values)(P<0.05). Serum FABP4,LCN2 levels in recurrence group were higher than those in remission group, and PON-1 levels were lower(P<0.05). There was statistical significance in prognostic risk stratification between recurrence group and remission group(P<0.05). Increased serum FABP4,increased LCN2,prognostic risk stratification, decreased serum PON-1 were indenpent risk factors for recurrence(P<0.05). The areas under curves(AUC) of serum FABP4,PON-1 and LCN2 single and combined determinations for predicting recurrence in AML patients after treatment were 0.883,0.910,0.906 and 0.978,respectively,and the combined determination was more effective than single determinations(Z=3.431,3.263 and 3.106,P<0.05). Conclusions The elevated levels of serum FABP4 and LCN2 and the decreased level of PON-1 in AML patients are related to their clinical pathological characteristics and recurrence after treatment. The combined judgment and prediction of these 3 indicators have certain value in predicting recurrence in AML patients after treatment.
