Role of combined determination of AVDL and LOI in short-term prognosis assessment of ACI patients

摘要:

目的 探究动静脉乳酸差值(AVDL)与乳酸氧合指数(LOI)联合检测在急性脑梗死(ACI)患者短期预后评估中的作用。方法 选取2023年7月—2025年6月南京医科大学附属脑科医院ACI患者500例,收集所有患者的临床资料,并检测动脉血和静脉血乳酸水平、动脉血氧饱和度和颈内静脉血氧饱和度,计算AVDL和LOI。对所有患者出院后随访28 d,采用改良Rankin量表(mRS)评分评估短期预后情况,并分为预后不良组(mRS评分3~6分,136例)和预后良好组(mRS评分0~2分,364例)。采用Pearson相关分析评估AVDL、LOI和入院24 h内的国立卫生研究院卒中量表(NIHSS)评分的相关性。采用二元Logistic回归分析评估ACI患者预后不良的影响因素。采用受试者工作特征(ROC)曲线评价各项指标单项检测和联合检测判断ACI患者预后不良的效能。结果 预后不良组脑梗死体积、D-二聚体(DD)、责任血管狭窄程度、入院NIHSS评分均高于预后良好组(P<0.05),AVDL和LOI绝对值均高于预后良好组(P<0.05)。AVDL、LOI和入院NIHSS评分均呈负相关(r值分别为-0.105、-0.098,P<0.05)。脑梗死体积较大、DD较高、责任血管狭窄程度较高、AVDL负值较大、LOI负值较大、入院NIHSS评分较高均是ACI患者预后不良的危险因素(P<0.05)。基于上述6项危险因素采用Logistic回归分析构建判断ACI患者预后不良的联合检测模型。脑梗死体积、DD、责任血管狭窄程度、AVDL、LOI、入院NIHSS评分单项检测和联合检测模型,以及剔除AVDL、LOI后的联合检测模型判断ACI患者预后不良的AUC分别为0.916、0.749、0.815、0.797、0.675、0.738、0.959、0.935。结论 ACI患者AVDL、LOI与神经功能缺损程度密切相关。基于脑梗死体积、DD、责任血管狭窄程度、AVDL、LOI、入院NIHSS评分构建的联合检测模型在ACI患者短期预后评估中有较高的临床价值。

Abstract:

Objective To investigate the role of combined determination of arteriovenous difference of lactate(AVDL)and lactic acid oxygenation index (LOI) in the short-term prognosis assessment of patients with acute cerebral infarction(ACI). Methods A total of 500 patients with ACI from the Affiliated Brain Hospital of Nanjing Medical University from July 2023 to June 2025 were enrolled. The clinical data were collected,and arterial and venous blood lactic acid levels,arterial blood oxygen saturation and internal jugular venous blood oxygen saturation were determined. AVDL and LOI were calculated. All the patients were followed up for 28 d after discharge,and the short-term prognosis was evaluated using the modified Rankin scale (mRS),and they were classified into poor prognosis group(mRS score 3-6,136 cases)and good prognosis group(mRS score 0-2,364 cases). Pearson correlation analysis was used to evaluate the correlation between AVDL,LOI and the National Institutes of Health Stroke Scale(NIHSS)score within 24 h after admission. Binary Logistic regression analysis was used to evaluate the risk factors for poor prognosis in ACI patients. The efficacy of single and combined determination models for judging poor prognosis in ACI patients was evaluated using receiver operating characteristic (ROC) curve. Results The volume of cerebral infarction,D-dimer(DD),stenosis degree of the responsible vessel and admission NIHSS score were higher in poor prognosis group than in good prognosis group (P<0.05),and the absolute values of AVDL and LOI were higher in poor prognosis group than those in good prognosis group(P<0.05). AVDL,LOI and admission NIHSS score were negatively correlated(r values were -0.105 and -0.098,P<0.05). Large cerebral infarction volume,high DD,high stenosis degree of the responsible vessel,large negative value of AVDL,large negative value of LOI and high admission NIHSS score were all risk factors for poor prognosis in ACI patients(P<0.05). Based on the above 6 risk factors,a combined determination model for judging poor prognosis in ACI patients was constructed using Logistic regression analysis. The areas under curves(AUC)of single and combined determination models of cerebral infarction volume,DD,stenosis degree of the responsible vessel,AVDL,LOI and admission NIHSS score,as well as the combined model without AVDL and LOI,for judging poor prognosis in ACI patients were 0.916,0.749,0.815,0.797,0.675,0.738,0.959 and 0.935,respectively. Conclusions AVDL and LOI are related to the degree of neurological dysfunction in ACI patients. The combined determination model constructed based on cerebral infarction volume,DD,stenosis degree of the responsible vessel,AVDL,LOI and admission NIHSS score has high clinical value in the short-term prognosis assessment of ACI patients.
