耐多药结核病患者治疗后血清YKL-40、NGAL和SOCS1水平对痰菌阴转的影响

Influence of serum levels of YKL-40,NGAL and SOCS1 in patients with multidrug-resistant tuberculosis after treatment on the conversion of sputum bacteria to negative state

摘要:

目的 探讨耐多药结核病(MDR-TB)患者治疗后血清甲壳质酶蛋白40(YKL-40)、中性粒细胞明胶酶相关脂质运载蛋白(NGAL)和细胞因子信号转导抑制物1(SOCS1)水平对痰菌阴转的影响。方法 选取2022年3月—2024年2月沧州市第三医院MDR-TB患者149例,收集所有患者的临床资料,并检测治疗前和治疗1个月后的血清YKL-40、NGAL和SOCS1水平。根据治疗6个月后的痰培养结果将患者分为痰菌阴转组(86例)和非痰菌阴转组(63例)。比较MDR-TB患者治疗前后血清YKL-40、NGAL、SOCS1水平差异。比较痰菌阴转组和非痰菌阴转组临床资料差异。采用多因素Logistic回归分析评估MDR-TB患者治疗后痰菌阴转的影响因素。采用受试者工作特征(ROC)曲线评价血清YKL-40、NGAL和SOCS1判断MDR-TB患者治疗后痰菌阴转的效能。结果 MDR-TB患者治疗后血清YKL-40、NGAL水平均低于治疗前(P<0.001),SOCS1高于治疗前(P<0.001)。痰菌阴转组和非痰菌阴转组治疗前血清YKL-40、NGAL、SOCS1水平差异均无统计学意义(P>0.05);治疗后,痰菌阴转组血清YKL-40和NGAL水平均低于非痰菌阴转组(P<0.001),血清SOCS1水平高于非痰菌阴转组(P<0.001)。痰菌阴转组耐药药物数量和糖尿病史、复治所占比例均低于非痰菌阴转组(P<0.05),其他临床资料2个组之间差异均无统计学意义(P>0.05)。血清YKL-40、NGAL升高和SOCS1降低是MDR-TB患者治疗后痰菌未阴转的危险因素(P<0.05)。血清YKL-40、NGAL、SOCS1单项检测和联合检测判断MDR-TB患者治疗后痰菌阴转的曲线下面积(AUC)分别为0.834、0.866、0.845、0.954。结论 MDR-TB患者治疗后血清YKL-40、NGAL降低,SOCS1升高,且与痰菌阴转密切相关。3项指标联合检测在MDR-TB患者治疗后痰菌阴转评估中有较高的价值。

Abstract:

Objective To investigate the influence of serum chitinase protein 40(YKL-40),neutrophil gelatinase-associated lipocalin(NGAL)and suppressor of cytokine signaling 1(SOCS1)levels on sputum bacterial negativity after treatment in patients with multidrug-resistant tuberculosis(MDR-TB). Methods A total of 149 MDR-TB patients from the Third Hospital of Cangzhou from March 2022 to February 2024 were enrolled. The clinical data were collected,and serum YKL-40,NGAL and SOCS1 levels were determined before and 1 month after treatment. According to the sputum culture results after 6 months of treatment,the patients were classified into sputum bacterial negativity group and non-sputum bacterial negativity group. The differences in serum levels of YKL-40,NGAL and SOCS1 in MDR-TB patients were compared before and after treatment. The differences in clinical data between sputum bacterial negativity group and non-sputum bacterial negativity group were compared. Multivariate Logistic regression analysis was used to evaluate the influencing factors of sputum bacterial negativity after MDR-TB treatment. Receiver operating characteristic(ROC)curve was used to evaluate the efficacy of serum YKL-40,NGAL and SOCS1 in judging sputum bacterial negativity after MDR-TB treatment. Results After treatment,the serum levels of YKL-40 and NGAL in MDR-TB patients were lower than those before treatment (P<0.001),while the level of SOCS1 was higher than that before treatment (P<0.001). Serum YKL-40 and NGAL levels in sputum bacterial negativity group and non-sputum bacterial negativity group before treatment showed no statistical significance(P>0.05). After treatment,serum YKL-40 and NGAL levels in sputum bacterial negativity group were lower than those in non-sputum bacterial negativity group(P<0.001),and serum SOCS1 levels were higher than those in non-sputum bacterial negativity group(P<0.001). The number of drug resistance drugs and diabetes history and the proportion of retreated cases in sputum bacterial negativity group were lower than those in non-sputum bacterial negativity group(P<0.05),while other clinical data showed no statistical significance between the 2 groups(P>0.05). Elevated serum YKL-40,NGAL and decreased SOCS1 were risk factors for non-sputum bacterial negativity after MDR-TB treatment(P<0.05). The areas under curves(AUC)of single and combined determinations of serum YKL-40,NGAL and SOCS1 for judging sputum bacterial negativity after MDR-TB treatment were 0.834,0.866,0.845 and 0.954,respectively. Conclusions Serum YKL-40 and NGAL decrease,and SOCS1 increases in MDR-TB patients after treatment,which are related to sputum bacterial negativity. The combined determination of the 3 indicators has certain value in the assessment of sputum bacterial negativity after MDR-TB treatment.
