    Laboratory examination analysis of 1 case of variability acute promyelocytic leukemia

    Objective To investigate the morphological changes and related laboratory examination of variability acute promyelocytic leukemia(APL) with high expressions of CD14 and CD7,and to improve the accuracy of variability APL and reduce misdiagnosis. Methods All clinical data and the results of laboratory examination were collected. Results The morphology of this patient was similar to that of acute myelocytic leukemia(AML)-M5. The nuclear distortion was obvious. The cytoplasm had fine particles. The binuclear abnormal promyelocytic cells were common. Immunological antibodies routinely expressed CD13,CD33,CD117 and cytoplasm myeloperoxidase(cMPO),accompanied by high expressions of CD14 and CD7. Qualitative screening of leukemia genes showed that PML-RARα fusion gene was positive,but NPM-MLF1 fusion gene was suspected positive. Conclusions APL is easy to mix with AML-M2 and AML-M5. Its morphology is similar to AML-M2. AML-M2 can be excluded by PML-RARαfusion gene-positive,cytogenetic t(15;17)(q22;q21) translocation. Some cell morphology is distorted and folded,and the cytoplasm has fine particles,which is similar to AML-M5. AML-M5 can be excluded by histochemical staining for peroxidase-positive and specific lipase-positive. It should synthesize clinical and laboratory examination to diagnose APL,and the diagnosis of APL is not affected by the expression of related antigens and the mutated fusion gene.
    Phenotype analysis of abnormal Hb Rush patients

    Objective To analyze the phenotype of abnormal hemoglobin(Hb) Rush. Methods The hematological phenotype characteristics of 15 sporadic cases of Z(8)Hb variants(Hb Rush)suggested by Hb electrophoresis were analyzed retrospectively,and thalassemia gene was determined. The sequence of beta globin gene was analyzed. Results In the 15 cases,abnormal Hb Rush bands(40.9%-58.2%) were determined by capillary electrophoresis,and HBB gene sequencing showed that all the cases carried the mutation of unstable Hb Rush mutations [HBB CD101 GAG>CAG,glutamic acid(Glu)>glutamine(Gln)]. Among the 15 cases,8 adults were Hb Rush pure carriers,and another 2 cases of Hb Rush with the mutation of alpha+thalassemia gene were basically normal in the blood analysis phenotype,while 3 children and 2 cases with congenital β-thalassemia presented mild to moderate anemia. Conclusions Hb Rush mutagenesis alone has an anemic hematological phenotype characteristic,which is easy to cause missed diagnosis and Hb electrophoresis is an economic and effective means to find abnormal Hb.
    Influence of age and sex on TEG parameters in healthy subjects

    Objective To study the influence of different ages and sex on thromboelastography(TEG) parameters[K(fibrinogen reaction time),R(clotting time),α angle(the grow rate of blood clot)and MA(maximum amplitude thrombus)],and to provide a reference for establishing appropriate reference intervals of TEG parameters. Methods A total of 2 935 healthy subjects were enrolled,and their TEG parameters were collected as research data. The cases were classified according to different ages and sex. The influence of different ages and sex on TEG parameters was evaluated. Results Among the 2 935 healthy subjects,there was no statistical significance for the parameter of K in different sex groups(P>0.05). There was statistical significance in different sex groups for the parameters of R,α angle and MA(P<0.01). Between <30-year-old and >40-year-old males,there was statistical significance for R,MA and α angle(P<0.01). Between 31-40-year-old and the other age females,there was statistical significance for K,MA and α angle(P<0.01). Between 31-40-year-old females and males,there was statistical significance for TEG parameters(P<0.01). Conclusions TEG parameters are influenced by various ages and sex. The reference intervals of TEG parameters in adults should be established practically.
    Role of bone marrow morphological examination in the bone marrow infiltration of non-Hodgkin's lymphoma

    Objective To evaluate the accuracy of bone marrow morphological examination in assessing bone marrow infiltration in non-Hodgkin's lymphoma(NHL),and to evaluate the roles of bone marrow aspiration and bone marrow biopsyin the detection of bone marrow infiltration of NHL. Methods Totally,159 bone marrow specimens of patients with NHL were analyzed retrospectively. Their bone marrow aspiration and bone marrow biopsyin results were analyzed. The sensitivity,specificity,consistency and correlation of bone marrow aspiration in the detection of bone marrow infiltration of NHL were compared with those of bone marrow biopsyin. Results The consistency of bone marrow aspiration and bone marrow biopsyin was 83.6%. The sensitivity and specificity of bone marrow aspiration were 53.8% and 98.1%,respectively. For the bone marrow infiltration of indolent NHL,the sensitivity and specificity of bone marrow aspiration were 73.7% and 92.0%,respectively,while the sensitivity and specificity for the bone marrow infiltration of invasive NHL were 42.4% and 100.0%,respectively. Conclusions bone marrow aspiration is a useful method to detect bone marrow infiltration in NHL. It might be used as a supplementary measurement for bone marrow biopsyin with the advantages of easiness,rapidness and cost-effectiveness.
    Correlations of RDW and TB with coronary heart disease and the extent of coronary atherosclerosis

    Objective To investigate the relations of red blood cell distribution width(RDW) and serum total bilirubin(TB) with coronary heart disease(CHD) and its coronary artery lesion branches and Gensini score. Methods A total of 200 CHD patients undergoing coronary angiography were enrolled as CHD group,taking positive coronary angiography(stenosis of major vessels ≥ 50%) as diagnostic standard of CHD. Totally,100 healthy subjects were enrolled as control group. The levels of RDW and TB were determined,and the relations of total cholesterol(TC),triglyceride(TG),high-density lipoprotein cholesterol(HDL-C),low-density lipoprotein cholesterol(LDL-C),cardiac troponin I(cTnI),myoglobin(MYO),B-type natriuretic peptide(BNP)and creatinine kinase-MB isoenzyme(CK-MB)with RDW and TB were analyzed by. Statistical analysis was used to analyze the difference between RDW and TB in patients with different coronary artery lesion branches. Spearman correlation analysis was used to analyze the relation between the levels of RDW and TB and Gensini score. Results RDW was higher in CHD group than that in control group(P<0.05),and the level of TB was decreased(P<0.05). In CHD group,RDW was negatively correlated with HDL-C(r=-0.677,P<0.05) and positively correlated with cTnI(r=0.196,P<0.01),and TB was positively correlated with HDL-C(r=0.525,P<0.05)and negatively correlated with cTnI(r=-0.118,P<0.05). RDW in patients with single-,double- and triple-branch coronary artery lesions increased in turn(P<0.01),and TB decreased in turn(P<0.01). RDW level was positively correlated with Gensini score(r=0.437,P<0.05). TB level was negatively correlated with Gensini score (r=-0.385,P<0.05). Conclusions RDW and TB are correlated with CHD and the severity of coronary artery lesions,and they play roles in the evaluation of CHD risk and prognosis.

    Objective To analyze the clinical distribution and trend of Acinetobacter baumannii(AB) and multidrug resistant Acinetobacter baumannii(MDR-AB) isolated in Shanghai Chest Hospital,and to provide a reference for controlling MDR-AB infection. Methods Totally,499 isolates of AB isolated in Shanghai Chest Hospital from 2015 to 2017 were identified by VITEK MS microbial mass spectrometry identification system. The drug susceptibility test in vitro was carried out by VITEK-2 COMPACT,and the related clinical data were analyzed. Results The detection rate of AB increased from 2015 to 2017(χ2=7.168,P<0.05),while the detection rate of MDR-AB decreased year by year(χ2=35.086,P<0.01). The detection rates of AB and MDR-AB in male patients were higher than those in female patients(P>0.05). The detection rates of AB and MDR-AB all reached the highest in patients of 60-69 years old,and there was a yearly upward trend of MDR-AB detection rate in patients of 70-79 years old(χ2=6.775,P<0.05). The specimens with AB and MDR-AB were mainly sputum specimens,most of which were from intensive care unit(ICU) of Shanghai Chest Hospital. Conclusions With the increasing of AB detection rate,the monitoring of MDR-AB should be strengthened,and disinfection and isolation should be done well to control the development of multidrug resistant isolates.
    HPV infection status of cervical cancer patients in Anhui and role of HPV determination in monitoring the curative effect of radiotherapy

    Objective To analyze the human papillomavirus(HPV) infection status and the subtype distribution of cervical cancer patients in Anhui,and to investigate the correlation between HPV infection and clinical pathological characteristics of cervical cancer patients and the relationship between HPV DNA copy number changes and the efficiency of radiotherapy. Methods A total of 230 patients with cervical cancer were enrolled. Real-time fluorescence quantitative polymerase chain reaction(PCR) was used to determine 15 common high-risk HPV DNA subtypes,and the positive rate and subtype distribution characteristics of HPV infection in patients with cervical cancer were analyzed. The relationship between positive rate and subtype distribution in ages and clinical stages and the relationship between the copy number changes of HPV DNA and the efficiency of radiotherapy in patients with cervical cancer were evaluated. Results The positive rate of HPV infection in 230 patients with cervical cancer was 83.0%,the single subtype positive rate was 73.2%,and the multiple subtype positive rate was 26.8%. The subtypes of HPV infection in cervical cancer patients were mainly HPV 16,18 and 58. HPV infection was not correlated with ages,but it was related to clinical stages(P<0.05). The later clinical stages made lower infection rates. The single subtype infection rate after radiotherapy was higher than that before radiotherapy,the radiotherapy efficiency of single subtype infection was higher than that of multiple subtype infection,and the copy number in invalid group would increase. Conclusions HPV infection in patients with cervical cancer in Anhui is dominated by HPV 16,18 and 58. HPV subtype and copy number of HPV infection could be used as the indicators for evaluating the efficiency of radiotherapy.
    Correlation between serum fetuin A level and insulin resistance in patients with hyperthyroidism

    Objective To investigate the relationship between serum fetuin A level and insulin resistance(IR) in hyperthyroidism patients. Methods A total of 60 patients with hyperthyroidism treated with carbimazole were enrolled,and these patients were all cured. Totally,60 healthy subjects were enrolled as healthy control group. The general data of all subjects(age,sex,height,weight and so on) were collected. The levels of fetuin A,thyroid stimulating hormone(TSH),free triiodothyronine(FT3),free thyroxine(FT4),insulin,C-reactive protein(CRP),serum calcium and blood glucose were determined before and after treatment in hyperthyroidism group and healthy control group. Pearson correlation analysis was used to evaluate the correlation between fetuin A and various indicators. Results Body mass index(BMI) and TSH in hyperthyroidism group before treatment were lower than those after treatment and healthy control group(P<0.05),and FT3,FT4,serum calcium,CRP,homeostasis model assessment for insulin resistance(HOMA-IR) and fetuin A were higher than those after treatment and healthy control group(P<0.05). There was no statistical significance for BMI,TSH,FT3,FT4,serum calcium,CRP and fetuin A between hyperthyroidism after treatment and healthy control group(P>0.05),but HOMA-IR was higher than that in healthy control group(P<0.05). Before and after treatment,serum fetuin A was positively correlated with FT3,FT4,CRP and HOMA-IR(P<0.05),and it was negatively correlated with TSH(P<0.05),but there was no correlation with BMI and serum calcium(P>0.05). Conclusions Elevated serum fetuin A level in hyperthyroidism patients may be related to IR.
    Analysis of urinary iodine levels of pregnant women in Qingdao

    Objective To investigate the iodine nutritional status of pregnant women in Qingdao,and to provide a reference for iodine intake for pregnant women properly. Methods Urinary levels of iodine and serum levels of free thyroxine(FT4) and thyroid stimulating hormone(TSH) of 484 pregnant women were determined,and the iodine nutritional status of pregnant women was evaluated according to the standards recommended by World Health Organization(WHO). Results Among the 484 pregnant women,pregnant women of urinary iodine <150 μg/L accounted for 19.42%,those of 150-<250 μg/L and >250 μg/L accounted for 27.07% and 53.51%,respectively. The proportions of iodine deficiency of early,middle and late pregnant women were 10.65%,21.14% and 28.93%,respectively. The proportion of iodine deficiency of late pregnant women was higher than that of early pregnant women(P<0.05). There was no statistical significance between middle and late pregnant women(P>0.05). The levels of TSH were higher in pregnant women with urinary iodine 150-<250 μg/L than in pregnant women with urinary iodine <150 μg/L(P<0.05). The levels of FT4 had no statistical significance among different urinary iodine groups(P>0.05). Conclusions The iodine nutritional status for pregnant women in Qingdao is fine,but still some individuals expose in iodine insufficiency. Iodine nutritional status in pregnant women has an effect on TSH levels. It's suggested that urinary iodine monitoring and thyroid function screening should be conducted in pregnant women,which could guide iodine intake for pregnant women properly.
    Establishment of the reference intervals of serum glutathione reductase in apparently healthy adults

    Objective To establish the reference intervals of serum glutathione reductase(GR) in apparently healthy adults. Methods Serum GR activities of 430 healthy adults,including 205 males and 225 females,were determined,and the age- and sex-related reference intervals were determined according to the Clinical and Laboratory Standards Institute(CLSI) C28-A3 document. Results After eliminating 5 outliers,the GR activities showed a non-Gaussian distribution in serum of healthy adults. Serum GR activity in males was 53(47-57) U/L,which was slightly higher than that in females [51(47-55) U/L](Z=-2.156,P<0.05). Correlation analysis showed that serum GR activities had positive correlations with age,alanine aminotransferase(ALT),aspartate aminotransferase(AST),gamma glutamyltransferase(GGT),alkaline phosphatase(ALP),glucose(Glu),triglyceride(TG),total cholesterol(TC) and low-density lipoprotein cholesterol(LDL-C)and a negative correlation with high-density lipoprotein cholesterol(HDL-C). There was no correlation between serum GR activities and total bilirubin(TB). The GR activities in males showed no statistical significance among all the age groups,and the subjects were combined into male group. The GR activities in ≥51-year-old females were higher than those in 18-50-year-old females,so that the subjects were regrouped into 18-50-year-old and ≥51-year-old groups. The reference intervals of serum GR activities in healthy adults were 36-72 U/L for males,38-65 U/L(18-50-year-old) and 45-67 U/L(≥51-year-old) for females by percentiles. Conclusions The reference intervals of serum GR in apparently healthy adults have initially established,which provides a reference for the clinical application of GR.
    Role of serum lipid transfer protein activity determination in patients with hypothyroidism

    Objective To investigate the role of serum lipid transfer protein activity determination in patients with hypothyroidism. Methods A total of 86 patients with hypothyroidism and 86 healthy subjects were enrolled. The general data [sex,age,smoking history,drinking history and body mass index(BMI)] were collected. Total cholesterol(TC),triglyceride(TG),low-density lipoprotein cholesterol(LDL-C),high-density lipoprotein cholesterol(HDL-C) and non-high-density lipoprotein cholesterol(non-HDL-C) levels and serum cholesterol ester transporter(CETP) and phospholipid transporter(PLTP) activities were determined. Multiple linear regression analysis was used to evaluate the relationship of hypothyroidism with blood lipids and lipid transfer proteins. Results Compared with healthy control group,serum HDL2-C level,HDL2-C/HDL3-C ratio,CETP activity and PLTP activity decreased(P<0.01),HDL3-C level increased(P<0.01),and there was no statistical significance in serum TC,TG,LDL-C,HDL-C and non-HDL-C between the 2 groups(P>0.05). CETP was positively correlated with HDL2-C(r=0.27,P<0.001),and it was negatively correlated with HDL3-C and thyroid stimulating hormone(TSH)(r=-0.22 and -0.21,P<0.01). PLTP was positively correlated with HDL2-C(r=0.37,P<0.001),and it was negatively correlated with HDL3-C and TSH(r=-0.24,P<0.001). There was no correlation of TSH with HDL2-C and HDL3-C(r=-0.291 and 0.012,P>0.05). Multiple linear regression analysis after adjusting for age,sex,BMI,smoking history and drinking history showed that CETP and PLTP were correlated with hypothyroidism(β=0.62 and 0.81,P<0.001),and they were not correlated with blood lipids(P>0.05). Conclusions The decrease of HDL2-C and the increase of HDL3-C in patients with hypothyroidism are related to the decrease of lipid transfer protein activity. These may be early biomarkers of lipid disorders in patients with hypothyroidism.
    Role of type 1 insulin-like growth factor receptor and epidermal growth factor receptor in endometrial carcinoma

    Objective To investigate the expressions of type 1 insulin-like growth factor receptor(IGF1R) and epidermal growth factor receptor(EGFR) in endometrial carcinoma(EC) and the relationship with clinicopathological characteristics. MethodsThe expressions of IGF1R and EGFR in 49 normal endometrial tissues,42 atypical endometrial hyperplasia tissues and 156 EC tissues were determiend by immunohistochemistry. The clinical data of 156 patients with EC were collected. The relationship between the expressions of IGF1R and EGFR and the clinicopathological characteristics of EC was analyzed. Pearson correlation analysis was used to evaluate the correlation between IGF1R and EGFR. Results The positive rates of IGF1R and EGFR in EC were higher than those in normal endometrial and atypical endometrial hyperplasia tissues(P<0.01). Pearson correlation analysis showed that IGF1R was positively correlated with EGFR(r= 0.618,P<0.01). The high expression of IGF1R in EC patients was related to the differentiation of EC,the International Federation of Gynecology and Obstetrics(FIGO) stage,classification,myometrial invasion,lymph node metastasis,vascular invasion and the expression of estrogen receptor(ER)(P<0.05),but there was no relation with age and progesterone receptor(PR)(P>0.05). The high expression of EGFR was correlated with the differentiation of EC,FIGO stage,classification and lymph node metastasis(P<0.05),but there was no relation with age,myometrial invasion,vascular invasion,ER expression and PR expression(P>0.05). The positive rate of lymph node metastasis in the group with high expressions of IGF1R and EGFR was higher than those in the groups with high expression of IGF1R or EGFR and low expressions of IGF1R and EGFR(P<0.05). Conclusions IGF1R and EGFR are correlated with poor clinicopathological characteristics of EC. The combined determination of IGF1R and EGFR may be used to determine lymph node metastasis in EC.
    Performance comparison of the detection of circulating tumor cells in hepatocellular carcinoma patients by qRT-PCR platform and CellSearch system

    Objective To compare the positive rates and correlation of CellSearch system and real-time quantitation polymerase chain reaction(qRT-PCR) platform in the detection of circulating tumor cells(CTC) in single-marker-[epithelial cell adhesion molecule(EpCAM)] and multi-marker-[EpCAM,CD133,CD90 and cytokeratin 19(CK19)] labeled patterns among hepatocellular carcinoma(HCC) patients. Methods The peripheral blood samples of 32 HCC patients were collected,and CTC were detected by CellSearch system and qRT-PCR platform(single-marker and multi-marker). The positive rates and correlation were analyzed using Spearman correlation analysis. Results The positive rate of CTC detection by single-marker(EpCAM) qRT-PCR platform was 46.9%(15/32),and that by multi-marker(EpCAM,CD133,CD90 and CK19) was 65.6%(21/32). The positive rate of CTC detection by CellSearch system was 28.1%(9/32). The consistency of positive rate between single-marker qRT-PCR platform and CellSearch system was 75.0%(24/32,P=0.002),that between multi-marker qRT-PCR platform and CellSearch system was 56.3%(18/32,P=0.034),and that between single-marker and multi-marker qRT-PCR platform was 81.3%(26/32,P=0.000). When using the qRT-PCR platform for multi-marker,the percentages of the 4 markers varied,and there existed obvious heterogeneity. Conclusions The multi-marker qRT-PCR platform can improve the positive rate of peripheral blood CTC detection in HCC patients,which directly reflects tumor heterogeneity and fits diverse detection demands.
    Role of gas chromatography-mass spectrometry in inherited metabolic diseases

After extraction and derivatization,132 metabolites were detected by GS-MS and analyzed by inherited metabolic disease analysis software. Combined with other laboratory tests,the diagnosis was performed. Results Of the 5 778 urine samples,1 100 cases had abnormal metabolites, accounting for 19%. A total of 106 cases with 17 diseases were diagnosed,among which 80 cases were diagnosed with abnormal characteristics in urine,and 26 cases were diagnosed with blood tandem mass spectrometry. Among the 80 cases,there were 65 cases with 11 types of organic acid metabolic disorders and 15 cases with 3 amino acid metabolic disorders,including 33 cases of methylmalonicmia,12 cases of hyperphenylalaninemia,7 cases of propionic acidemia,7 cases of glutaric acidemia typeⅠ,6 cases of urea circulatory disorders,3 cases of β-ketothiolase deficiency,2 cases of methylmalonic acidemia,2 cases of maple syrup urine disease,2 cases of isovaleric acidemia,2 cases of multiple carboxylase deficiency,1 case of tyrosinemia type Ⅰ,1 case of 2-hydroxyglutaratemia,1 case of 3-methylcrotonyl-CoA carboxylase deficiency and 1 case of 3-hydroxy-3-methylglutaric acidemia. Totally,26 cases were diagnosed by tandem mass spectrometry,including 19 cases of Citrin deficiency,5 cases of very long-chain acyl-CoA dehydrogenase deficiency and 2 cases of medium-chain acyl-CoA dehydrogenase deficiency. Conclusions GC-MS is specific for the abnormality of organic acids and partial amino acid metabolism abnormality. The results of GC-MS are influenced by many factors,but it has no difference in clinical judgment.
    Application of root cause analysis for improving bar code recognition rate in clinical laboratories

    Objective To investigate the role of root cause analysis(RCA) in improving the recognition rate of bar code in clinical laboratories. Methods RCA was used to analyze the root causes of frequent failures of bar code recognition and the bidirectional transmission of large-sized instruments. The corrective measures were formulated,which was compared with that before corrective measures. Results Bar code recognition rate was improved after RCA implementing(P<0.05). Conclusions RCA plays a role in improving the recognition rate of bar code in clinical laboratories,and it can sort out the key points of adverse events. The continuous improvement of the quality of work before inspection and analysis is realized.

    Human microbiome,the collective genome of the microbial community that is on and within us,has recently been mapped with sequencing technologies. Recent metagenomic studies have shown that the vaginal microbiome varies among reproductive age women. Moreover,during pregnancy,vaginal microbiome may also change with hormonal fluctuations. Pregnancy complications,such as preeclampsia and premature birth,were also associated with microbiome dysbiosis. This review provides a brief overview on observations pertaining to both variation and the presumptive potential role of a varied microbiome during pregnancy,the effects of dysbacteriosis on major pregnancy complications and the usage of next-generation sequencing technology as a means to investigate the microbiome.

    Quantum mechanics is a branch of physics,which studies the laws of motion of particles at small scales and atoms at low energy levels. As a result of the rapid development of quantum theory and progress in instruments and techniques,various quantum techniques based on quantum theory are widely used in different disciplines,including quantum computing,quantum communication,quantum metrology,quantum imaging,quantum dot luminescence and computer-aided drug design,which makes scientific researches more convenient. Selected applications for quantum mechanics are given in this review mainly focusing on the perspective of biomedical science.
