The significance of IGF-1 and IGFBP-3 in the diagnosis of gastric cancer

Abstract: Objective To evaluate the clinical significance of serum insulin-like growth factor-1(IGF-1) and insulin-like growth factor binding protein-3(IGFBP-3) in the diagnosis of gastric cancer and gastric precancerous lesion. Methods The serum levels of IGF-1,IGFBP-3,carcinoembryonic antigen (CEA) and carbohydrate antigen 199(CA199) were measured by chemiluminescence immunoassay,enzyme-linked immunosorbent assay(ELISA) and electro-chemiluminescence immunoassay in 90 patients with gastric cancer,40 patients with gastric benign disease and 50 healthy subjects. Results The serum levels of IGF-1 in patients with gastric cancer were significantly higher than those in patients with gastric benign disease and healthy subjects(P<0.05,P<0.001). The serum levels of IGFBP-3 in patients with gastric cancer were significantly lower than those in patients with gastric benign disease and healthy subjects(P<0.001).The serum levels of IGF-1 in the early (Ⅰ-Ⅱ) stage of gastric cancer were lower than those in the late (Ⅲ-Ⅳ) stage, but were higher than those in the healthy subjects,and the serum levels of IGFBP-3 were higher than those in the late stage, but were lower than those in the healthy subjects.The serum levels of IGF-1 had significantly negative correlation with IGFBP-3,and the linear correlation equation was Y =-0.33X +116.65(r=-0.566,P<0.01). There was no correlation between IGF-1 and IGFBP-3 with CEA and CA199. Using the healthy subjects as controls,the areas under the receiver operating characteristic(ROC) curve (AUC) of IGF-1  was obviously higher than those of CEA and CA199(P<0.01,P<0.05). The sensitivity of IGF-1 for the diagnosis of gastric cancer was higher than those of CEA and CA199(P<0.05). Conclusions IGF-1 is better than CEA and CA199, in the diagnosis of gastric cancer,and it is helpful for the evaluation of gastric cancer stages.
