    Difference in identification of non-tuberculous Mycobacterium between microarray chip and multiple homologous sequence

    Objective To compare microarray chip and multiple homologous sequence in the identification of non-tuberculous Mycobacterium(NTM),and to provide accurate diagnosis reference for the clinical diagnosis and treatment of NTM infection. Methods For preserved NTM from 2016 to 2019 in Tianjin Haihe Hospital,using microarray chip and multiple homologous sequence,including 16S rRNA,subunit of RNA polymerase B(rpoB),heat shock protein 65(hsp65),the 170 isolates of NTM were analyzed. The accuracy of microarray chip was analyzed for NTM using the identification results of multiple homologous sequence as reference method. The turn-around times(TAT) were compared between the 2 methods. Results Totally,8 NTM species were identified by microarray chip,and 14 NTM species were identified by multiple homologous sequence. Microarray chip failed to identify 10 isolates of slowly growing MycobacteriaMycobacterium simiaeMycobacterium lentflavum and Mycobacterium parascrofulaceu) and rapidly growing MycobacteriaMycobacterium chelonaeMycobacterium mageritense and Mycobacterium vaccae). All the isolates could be identified by multiple homologous sequence. Based on the results of multiple homologous sequence,7 errors were identified by microarray chip,and the identification accuracy of complex groups was 90.0%(153/170). The accuracies of 16S rRNA,rpoBhsp65 and 3 homologous sequences were 93.5%,98.8%,99.4% and 100.0%,respectively. For 53 isolates of Mycobacterium chelonis/Mycobacterium abscessus complex(MABC) that could not be distinguished by microarray chip,6 isolates of Mycobacterium chelonis,25 isolates of MABC subspecies,20 isolates of MABC Masai subspecies and 2 isolates of MABC bolletii subspecies were identified by 3 homologous sequences. It took 8 h for TAT to be identified by microarray chip and 2 d for all the 170 NTM isolates to be identified. It took 3 d to identify TAT from a single sample by multiple homologous sequence and about 5 d to identify all the 170 NTM isolates. Conclusions Microarray chip could quickly provide accurate results for clinical identification of common Mycobacterium species. Multiple homologous sequence could not only identify common Mycobacterium species,but also could provide more accurate identification results and subspecies for the isolates that can not be identified by microarray chip.

    Application of nucleic acid aptamer fluorescence probe method and liquid-based sandwich cup method in the determination of Mycobacterium tuberculosis

    Objective To compare the clinical application values of nucleic acid aptamer fluorescence probe method and liquid-based sandwich cup method in the determination of Mycobacterium tuberculosis(MTB). Methods Sputum samples from 175 patients with pulmonary tuberculosis were collected and determined by liquid culture method,solid culture method,acid fast staining,liquid-based sandwich cup method and nucleic acid aptamer fluorescence probe method. Liquid culture method was used as the reference standard. The consistency of the results of nucleic acid aptamer fluorescence probe method,liquid-based sandwich cup method and liquid culture method was evaluated. Results Compared with the results of liquid culture method,the sensitivity of liquid-based sandwich cup method was 78.08%,and the specificity was 98.04%. The sensitivity of nucleic acid aptamer fluorescence probe method was 54.79%,and the specificity was 50.98%. Conclusions Liquid-based sandwich cup method is consistent with liquid culture method,and it has good clinical diagnostic value. Nucleic acid aptamer fluorescence probe method needs to be further optimized.

    Relationship between serum cathepsin K and ADAMTS-7 with ventricular remodeling and prognosis in patients with chronic heart failure

    Objective To investigate the relationship between serum cathepsin K(Cat-K) and a disintegrin and metalloproteinase with thrombospondin motifs 7(ADAMTS-7) with ventricular remodeling and prognosis in chronic heart failure(CHF) patients. Methods Totally,195 patients with CHF(CHF group) and 101 healthy subjects(control group) were enrolled from Handan Central Hospital from February 2018 to June 2020. The general data of CHF patients were collected. Random blood glucose,glycated hemoglobin A1c(HbA1c),N-terminal pro-B-type natriuretic peptide(NT-proBNP),B-type natriuretic peptide(BNP),Cat-K and ADAMTS-7 levels and left ventricular ejection fraction(LVEF),ventricular remodeling parameters [left ventricular end-diastolic diameter(LVEDD),left ventricular posterior wall thickness(LVPWT),interventricular septum thickness(IVST)] were determined,and left ventricular mass index(LVMI) was calculated. According to New York Heart Association(NYHA) classification,the CHF group was classified into Grade Ⅱ group(65 cases),Grade Ⅲ group(82 cases) and Grade Ⅳ group(48 cases). Pearson correlation analysis was used to evaluate the correlation among the indicators. Logisitc regression analysis was used to evaluate the risk factors for poor prognosis in CHF patients. Receiver operating characteristic(ROC) curve was used to evaluate the efficacy of each indicator in determining poor prognosis in patients with CHF. Results Serum Cat-K and ADAMTS-7 in CHF group were higher than those in control group(P<0.05). Serum Cat-K,ADAMTS-7,BNP,NT-proBNP,LVEDD,LVPWT,IVST and LVMI in NYHA Grade Ⅳ group were higher than those in NYHA Grade Ⅲ group and NYHA Grade Ⅱ group(P<0.05). LVEF was lower than those of NYHA Grade Ⅲ group and NYHA Grade Ⅱ group(P<0.05). Pearson correlation analysis showed that Cat-K and ADAMTS-7 were positively correlated with BNP,NT-proBNP,LVEDD,LVPWT,IVST and LVMI(P<0.05),and they were negatively correlated with LVEF(P<0.05). Logistic regression analysis showed that the increasing of NT-proBNP,NYHA grade,Cat-K and ADAMTS-7 and the decreasing of LVEF were risk factors for poor prognosis in CHF patients [odds ratio(OR)=1.057,1.772,1.075,1.335 and 0.903,95% confidence intervals(CI) were 1.003-1.113,1.159-2.709,1.034-1.116,1.091-1.634 and 0.819-0.996,respectively]. The area under curve (AUC) of NT-proBNP,LVEF,NYHA grade,Cat-K and ADAMTS-7 in determining poor prognosis of CHF patients were 0.729,0.704,0.803,0.720,0.688 and 0.886,respectively. Conclusions Serum Cat-K and ADAMTS-7 are correlated with ventricular remodeling and poor prognosis in CHF patients.

    Expression of PBX1 in osteosarcoma tissues and its influence on prognosis

    Objective To investigate the correlation of the expressions of pre-B-cell leukemia transcription factor 1(PBX1) gene and protein in osteosarcoma tissues with clinicopathologic parameters of patients and its influence on prognosis. Methods A total of 85 patients with osteosarcoma from the Second People's Hospital of Jiaozuo City from January 2013 to January 2016 were enrolled. The expressions of PBX1 protein and mRNA in osteosarcoma tissues and normal bone tissues(>5 cm from tumor edges) were determined,and the clinical data of patients were collected. The patients were followed up on the first day after surgery and were followed up for 5 years. Kaplan-Meier survival curve was used to analyze the survival status of patients with osteosarcoma. Cox proportional hazard regression model was used to analyze the influencing factors for prognosis in patients with osteosarcoma. Results The positive rate of PBX1 protein and the relative expression of mRNA in osteosarcoma tissues were higher than those in normal bone tissues(P<0.001). The positive rate of PBX1 protein and the mRNA relative expression in tumor tissues of patients with Enneking stage ⅡB-Ⅲ and osteosarcoma with distant metastasis were higher than those in Enneking stage Ⅰ-ⅡA and osteosarcoma without distant metastasis(P<0.05). Kaplan-Meier survival curve analysis showed that the mean survival time and 5-year survival rate of patients with PBX1 protein positive were lower than those of patients with PBX1 protein negative(P<0.001). The results of Cox proportional hazard regression model showed that Enneking stage ⅡB-Ⅲ,distant metastasis and PBX1 protein positive were risk factors affecting the survival of patients with osteosarcoma [hazard ratios(HR) were 2.861,2.244 and 2.361,95% confidence intervals(CI) were 1.204-6.799,1.047-4.814 and 1.258-5.265,respectively]. Conclusions The protein positive rate and mRNA expression of PBX1 in osteosarcoma tissues are increased,and they are related to Enneking stage and distant metastasis,which may be used as potential biomarkers for early diagnosis and prognosis evaluation of patients with osteosarcoma.

    Expressions of serum miR-24 and XIAP mRNA in serum of oral adenoid cystic carcinoma patients and relationship with prognosis

    Objective To investigate the expression levels of serum microRNA-24(miR-24) and X-linked inhibitor of apoptosis protein(XIAP) mRNA in patients with oral adenoid cystic carcinoma(ACC) and the relationship with prognosis. Methods Totally,60 patients with oral ACC treated in Zhangjiakou First Hospital from June 2010 to February 2016 were enrolled as oral ACC group,and 60 healthy subjects during the same period were enrolled as healthy control group. Serum samples from all the subjects were collected. Cancer and para-cancer(≥5 cm from tumor tissue margin) tissue samples and clinical data were collected. The expression levels of miR-24 and XIAP mRNA in serum of the 2 groups were determined. The relationship between the expression levels of serum miR-24 and XIAP with the clinicopathologic characteristics of oral ACC patients were analyzed by pearson method. All the ACC patients were followed up. Kaplan-Meier survival curve and Log-rank χ2 test were used to analyze the relationship between the relative expression levels of serum miR-24 and XIAP mRNA and the prognosis of patients with oral ACC. Cox regression analysis was used to evaluate the prognostic risk factors of patients with oral ACC. Results Compared with healthy control group,the relative expression level of serum miR-24 in oral ACC group was decreased(P<0.05),and the relative expression level of XIAP mRNA was increased(P<0.05). Compared with para-cancer tissues,the relative expression of miR-24 in cancer tissues was decreased(P<0.05),and the relative expression of XIAP mRNA was increased(P<0.05). There was a negative correlation between serum miR-24 and XIAP mRNA in oral ACC patients(r=-0.486,P<0.05). Oral ACC patients were classified into high expression group and low expression group using the mean values of serum miR-24 and XIAP mRNA relative expression levels as cut-off values. The numbers of nerve invasion and lymphatic metastasis in miR-24 low expression group and XIAP mRNA high expression group were higher than those in miR-24 high expression group and XIAP mRNA low expression group,respectively(P<0.05). Kaplan-Meier survival curve analysis showed that the cumulative survival rate of miR-24 low expression group and XIAP mRNA high expression group was lower than that of miR-24 high expression group and XIAP mRNA low expression group(Log-rank χ2 values were 7.212 and 8.568,P=0.007 and 0.003,respectively). Cox regression analysis showed that nerve invasion and high-expressed XIAP mRNA were independent risk factors for poor prognosis in oral ACC patients(P<0.05),and high-expressed miR-24 was independent protective factor for oral ACC patients(P<0.05). Conclusions Serum miR-24 and XIAP mRNA in patients with oral ACC are correlated with prognosis,and may be used as new biomarkers to evaluate the prognosis of oral ACC.

    Determination and clinical significance of NLRP3,CTRP6 and IL-1β in patients with gestational diabetes mellitus

    Objective To investigate the clinical significance of serum NOD-like receptor protein(NLRP3),C1q/tumor necrosis factor-related protein 6(CTRP6) and interleukin 1 beta(IL-1β) in patients with gestational diabetes mellitus(GDM). Methods Totally,145 patients with GDM in Nantong Maternal and Child Health Care Hospital from January 2018 to December 2020 were enrolled as GDM group,and 145 healthy pregnant women were enrolled as control group. Serum NLRP3,CTRP6,IL-1β levels and fasting insulin(FINS),glycated hemoglobin A1c(HbA1c),fasting plasma glucose(FPG),homa-beta cell function index(HOMA-β),homeostasis model assessment of insulin resistance(HOMA-IR) levels were determined. According to the pregnancy outcomes,they were classified into adverse pregnancy outcome group and normal pregnancy outcome group. Pearson correlation analysis was used to evaluate the correlation between various indicators in GDM group. Receiver operating characteristic(ROC) curve was used to evaluate the effectiveness of all the indicators in determining adverse pregnancy outcomes. Results Compared with control group,CTRP6,NLRP3,IL-1β,FINS,HbA1c,FPG and HOMA-IR levels in GDM group were increased(P<0.05),while HOMA-β was decreased(P<0.05). CTRP6,NLRP3 and IL-1β were positively correlated with FINS,HbA1c,FPG and HOMA-IR(P<0.05),and they were negatively correlated with HOMA-β levels(P<0.05). Compared with normal pregnancy group,CTRP6,NLRP3,IL-1β,FINS,HbA1c,FPG and HOMA-IR were increased in adverse pregnancy group(P<0.05),while HOMA-β was decreased(P<0.05). The areas under curves(AUC) of FINS,HbA1c,FPG,HOMA-β and HOMA-IR for judging adverse pregnancy outcome were all <0.7,while the AUC of CTRP6,NLRP3 and IL-1β for judging adverse pregnancy outcome were 0.846,0.802 and 0.871,respectively. The AUC of combined determination was 0.931. Conclusions The serum levels of CTRP6,NLRP3 and IL-1β in patients with GDM are increased,and they are closely related to insulin resistance and blood glucose levels. The combined determination of CTRP6,NLRP3 and IL-1β has high clinical value in predicting adverse pregnancy outcome.

    Predictive value of sd-LDL-C/LDL-C ratio for in-stent restenosis in patients with AMI after interventional therapy

    Objective To investigate the predictive value of small and dense low-density lipoprotein cholesterol(sd-LDL-C)/low-density lipoprotein cholesterol(LDL-C) ratio for in-stent restenosis in patients with acute myocardial infarction(AMI) after interventional therapy. Methods A total of 145 patients with AMI who underwent percutaneous coronary intervention(PCI) in Inner Mongolia Autonomous Region People's Hospital from August 2020 to August 2021 were enrolled. The clinical data of all the patients were collected,and the levels of serum total cholesterol(TC),triglyceride(TG),LDL-C,high-density lipoprotein cholesterol(HDL-C),sd-LDL-C and C-reactive protein(CRP) were determined at admission,and the ratio of sd-LDL-C/LDL-C was calculated. The patients were followed up for 1 year after PCI,and the occurrence of in-stent restenosis in AMI patients was evaluated according to the results of coronary angiography. The correlation between sd-LDL-C/LDL-C ratio and in-stent restenosis in AMI patients was evaluated by point two-column correlation analysis. Logistic regression analysis was used to evaluate the risk factors of in-stent restenosis in AMI patients. Receiver operating characteristic(ROC) curve was used to evaluate the efficacy of sd-LDL-C/LDL-C ratio in judging in-stent restenosis in AMI patients. Results During the 1-year follow-up after PCI,4 patients were lost to follow-up. Among the remaining 141 patients,20 patients(14.18%) had in-stent restenosis(stenosis group),and the other 121 patients without stenosis were enrolled into non-stenosis group. The LDL-C,sd-LDL-C and LDL-C/sd-LDL-C ratio in stenosis group were higher than those in non-stenosis group(P<0.05). There was no statistical significance in HDL-C and CRP between the 2 groups(P>0.05). The occurrence of in-stent restenosis in AMI patients was positively correlated with sd-LDL-C,LDL-C and sd-LDL-C/LDL-C ratio(r=0.441,0.351 and 0.347,P<0.001). The increased sd-LDL-C,LDL-C and sd-LDL-C/LDL-C ratio were risk factors for in-stent restenosis in AMI patients(P<0.05). The area under curve(AUC) of serum sd-LDL-C,LDL-C and sd-LDL-C/LDL-C ratio in judging in-stent restenosis in AMI patients were 0.709,0.706 and 0.750,respectively. Conclusions The levels of sd-LDL-C and LDL-C are related to the occurrence of in-stent restenosis after PCI in AMI patients. The ratio of sd-LDL-C/LDL-C may be used as an index to judge in-stent restenosis.

    Monitoring and analysis on measles antibody levels in females aged 20-40 in Dali Bai Autonomous Prefecture from 2016 to 2020

    Objective To investigate measles antibody levels in females aged 20-40 in Dali Bai Autonomous Prefecture,and to formulate a measles vaccination strategy and provide scientific evidence for children under 8 months. Methods Totally,4 908 females in Dali Bai Autonomous Prefecture were enrolled from 2016 to 2020,and measles IgG antibody was determined. The geometric mean concentration(GMC) of measles antibodies was calculated. Administrative region,ethnicity,age,region(urban,rural),measles containing vaccine(MCV) immunization history and measles history were collected. When the concentration of measles IgG antibody was ≥250 mIU,it was considered positive. When measles IgG antibody reached ≥800 mIU,it was considered to provide protection against measles. Multivariate Logistic regression model was used to analyze the influence factors for measles antibody positivity. Results Through an epidemiological survey of 4 908 females from 20 to 40 years old,the measles antibody positive rate was 91.4%,the protection rate was 63.1%,and the measles antibody GMC was 1 010.25 mIU/mL. There was statistical significance in the positive rate of measles antibody,measles antibody protection rate and measles antibody GMC between different years and administrative districts(P<0.05). There was statistical significance in the positive rate of measles antibody,measles antibody protection rate and measles antibody GMC among different ethnic groups and MCV immunization history(P<0.05). There was no statistical significance in the positive rate and protection rate of measles antibody among different age groups and regions(P>0.05). The difference in measles antibody GMC for females was statistically significant among different age groups(P<0.05). There was no statistical significance in measles antibody GMC between urban and rural(P>0.05). There was no statistical significance for the positive rate of measles antibody,the protection rate of measles antibody and the GMC of measles antibody whether there was measles history(P>0.05). Multivariate Logistic regression analysis showed that ethnicity and MCV immunization history were influence factors. The odds ratios(OR) were 0.896 and 1.776,and 95% confidence intervals(CI) were 0.842-0.952 and 1.425-2.214,respectively. Conclusions Increasing the vaccination rate of Dali Bai Autonomous Prefecture's females to MCV can effectively reduce the risk of measles infection in children under 8 months.

    Objective To investigate the predictive value of red blood cell distribution width(RDW) for in-hospital mortality in intensive care unit(ICU) patients with heart failure(HF). Methods A retrospective study was conducted on adult patients(≥18 years old) diagnosed with HF from 2001 to 2012 in the medical information mart for intensive care(MIMIC-Ⅲ) database. Receiver operating characteristic(ROC) curve,multivariate Logistic regression analysis and net reclassification index(NRI) were used to investigate the prognostic value of RDW. Results Totally,1 177 patients who met the inclusion criteria were enrolled. After adjustment for potential confounders,high RDW was correlated with in-hospital mortality [odds ratio(OR)= 1.578,95% confidence interval(CI) 1.019-2.445,P=0.041]. The area under curve(AUC) of RDW for predicting in-hospital mortality among HF patients in ICU was 0.633. Compared with GWTG-HF alone,the NRI of RDW combined with GWTG-HF was 0.325(95%CI 0.056-0.623,P=0.021). Conclusions RDW can be used to predict the in-hospital mortality of HF patients in ICU. RDW combined with GWTG-HF may increase the predictive value of in-hospital mortality.

    Establishment,verification and clinical application of automatic urine analysis system audit rule

    Objective To establish an automatic urine analysis system audit rule and evaluate its clinical application effects. Methods Totally,112 753 urine samples were collected at the First Affiliated Hospital of Air Force Military Medical University from April 2019 to August 2020. According to the WS/T 616—2018 "Automatic Audit of Clinical Laboratory Quantitative Inspection",urine analysis line(2 urine dry chemical analyzers and 3 urine visible composition analyzers) operation platform was used. In combination with the self-established relevant determination system,the urine determination reference interval,hemoglobin(Hb),red blood cell(RBC),leukocyte esterase(LEU) and white blood cell(WBC) grade corresponding confirmation relationship was determined. With the results of artificial microscopy as the gold standard,the audit rules of urine analysis line operation platform were established. The validity of audit rule was verified by true negative rate,false negative rate,true positive rate,false positive rate,microscopy rate,automatic audit and manual audit agreement rate(correct rate). The clinical effectiveness was evaluated according to audit time. Results A total of 25 automatic urine analysis system audit rules and Delta Check rules were established. Validity verification results showed that the true negative rate was 44.03%(487/1 106),the false negative rate was 4.52%(50/1 106),the true positive rate was 29.75%(329/1 106),the false positive rate was 21.70%(240/1 106),and the microscopy rate was 33.54%(371/1 106). The agreement rate between automatic audit and manual audit was 100.00%(7 338/7 338). The results of clinical effectiveness evaluation showed that the total pass rate during the trial operation period from May to August 2020 was 65.0%. Compared with May to August 2019,the median audit times of non-microscopy samples and microscopy samples were decreased by 10.3 and 9.3 min from 8:00 to 11:00 daily from May to August 2020,the audit effectivenesses were increased by 47.0% and 18.3%,and the number of auditors was decreased by 1,respectively. Conclusions The established automatic urine analysis audit rule can ensure the quality requirements,improve work efficiency and have good clinical applicability.

    Analysis of homocysteine trueness verification results in 2021

    Objective To evaluate the analytical quality of homocysteine(Hcy) in Shanghai clinical laboratories,through analyzing the results of Hcy trueness verification of 2021,Shanghai. Methods Residual samples from clinical laboratory tests were collected and prepared into high and low concentrations of frozen mixed serum samples(HCY2021-A and HCY2021-B). Participating clinical laboratories measured the samples for several times in 3 consecutive working days and reported the results through the network platform. The deviation between clinical laboratory test data and target value,coefficient of variation(CV),deviation and CV after grouping by different reagents were analyzed. Results The CV of HCY2021-A and HCY2021-B were 0.28%-9.44% and 0.25%-10.69%,respectively. The deviations between HCY2021-A and target value were -20.21%-31.39%. The deviations between HCY2021-B and target value were -29.66%-23.93%. Grouped by reagents,the deviations between the mean value of HCY2021-A and target value were -25.33%-20.27%,and the deviations between the mean value of HCY2021-B and target value were -18.41%-23.00%. The qualified rates of clinical laboratories with the results of reference method as the target value and the standard of 1/2 allowable total error(TEa) were 62.07% and 65.52%,respectively. Conclusions The accuracy of Hcy determination in Shanghai clinical laboratories remains to be improved.

    Analysis of chronic mast cell leukemia secondary to hematopoietic stem cell transplantation in myelodysplastic syndrome

    Objective To analyze the morphology and prognosis correlation of chronic mast cell leukemia after allogeneic hematopoietic stem cell transplantation in patients with myelodysplastic syndrome. Methods The clinical manifestations and treatment process of this case were analyzed retrospectively,and the laboratory determination results were analyzed. Results The patient had no mast cells before transplantation,and all the laboratory determination results after fully compatible transplantation met the diagnostic criteria for mast cell leukemia. The determination results of chimerism rate indicated that the patient was in a complete chimerism state at the early stage of transplantation. With the increase of the proportion of mast cells in the bone marrow,the chimerism rate was decreased,and the patient was in a mixed chimerism state. Conclusions Relatively mature mast cells may suggest a better prognosis. Systemic mast cell hyperplasia occurs on the basis of clonal hematopoietic tissue diseases,which can easily be ignored and lead to disease progression. Therefore,mast cell proliferation should be vigilant when patients with hematological tumors repeatedly develop allergic symptoms such as cough.

    Gastric cancer is the fifth most common malignancy worldwide and has the third highest mortality rate among cancer-related deaths. Epstein-Barr virus(EBV) infection is highly prevalent in the population and is known to be associated with various human cancers,such as nasopharyngeal cancer,gastric cancer and lymphoma. The Epstein-Barr virus-associated gastric cancer(EBVaGC) accounts for 1.3%-30.9% of all gastric cancers. Many non-coding RNA(ncRNA) have been shown to be involved in the regulation of proliferation,migration and invasion processes in EBV-associated cancers,and the interaction between ncRNA and potential target genes is gradually becoming a research hot-spot. Therefore,this review focuses on the research progress of ncRNA related to migration and invasion in gastric cancer,with a view to providing a reference for subsequent research on gastric cancer.

    Periprosthetic joint infection(PJI) requires timely and accurate diagnosis,and current diagnostic criteria can not 100% identify all grades of infection. Traditional biomarkers,such as C-reactive protein,are still widely used as the first choice for diagnosing periprosthetic infection,when history and physical examination are clear. However,with the research on periprosthetic infection,new biomarkers and determination technologies continue to emerge,making the diagnosis of periprosthetic infection continue to improve. This review focuses on the research progress of periprosthetic infection-related biomarkers and determination technologies.
